医养护一体化管理模式在社区老年慢性病患者管理中的应用价值

摘　　要：目的:分析医养护一体化管理模式在社区老年慢性病患者管理中的应用效果。方法:选取2019年6月-2020年12月在社区服务中心登记的100例老年慢性病患者,接受社区一般管理的50例老年患者纳入对照组,接受医养护一体化管理的50例老年患者作为观察组。比较两组认知水平、治疗依从性、管理满意度。结果:管理后,两组认知功能均较管理前提升,观察组认知功能评分明显高于对照组,差异有统计学意义(P<0.05);观察组治疗总依从性为98.0%,管理总满意度为96.0%;对照组治疗总依从性为76.0%,管理总满意度为72.0%,差异均有统计学意义(P<0.05)。结论:社区老年慢性病患者接受医养护一体化管理后,健康情况明显提升,不仅强化了老年人对慢性病的认知程度,还在很大程度上提升了老年人的治疗依从性,获得了许多患者的满意,具有积极的推广价值。Objective:To analyze the application effect of the integrated management model of medical care and nursing in the management of elderly patients with chronic diseases in the community.Method:A total of 100 elderly patients with chronic diseases registered in the community service center from June 2019 to December 2020 were selected,50 elderly patients receiving general community management were included in the control group,and 50 elderly patients receiving integrated management of medical care and nursing were selected as the observation group.The cognition level,treatment compliance,and management satisfaction of the two groups were compared.Result:After management,the cognitive function of the two groups was improved than that before management,and the cognitive function score of the observation group was significantly higher than that of the control group,the difference was statistically significant (P<0.05).The total treatment compliance and management satisfaction of the observation group were 98.0% and 96.0% respectively,the total treatment compliance was 76.0% and the total management satisfaction was 72.0% in the control group,with statistically significant differences (P<0.05).Conclusion:After the elderly patients with chronic diseases in the community received the integrated management of medical care and nursing,their health status is significantly improved,which not only strengthened the elderly’s awareness of chronic diseases,but also greatly improved the elderly’s compliance with treatment,which has obtained the satisfaction of many patients and has positive promotion value.
