腹腔镜辅助经肛全直肠系膜切除术与腹腔镜全直肠系膜切除术治疗超低位直肠癌的临床价值比较

摘　　要：目的:探讨腹腔镜全直肠系膜切除术(TME)及腹腔镜辅助经肛门全直肠系膜切除术(taTME)治疗超低位直肠癌的效果。方法:选取笔者所在医院2018年7月-2020年7月收治的60例超低位直肠癌患者作为本次研究对象,将其随机分为两组,每组30例。一般组采用腹腔镜TME治疗,试验组采用腹腔镜辅助taTME治疗。比较两组治疗效果。结果:试验组手术时间长于一般组,但排气时间、住院时间、术中出血量均优于一般组,差异有统计学意义(P<0.05)。试验组治疗后PAC-QOL评分明显优于一般组,差异有统计学意义(P<0.05)。两组术后并发症发生率比较差异无统计学意义(P>0.05)。两组1年生存率、局部复发、远处转移率比较差异均无统计学意义(P>0.05)。结论:与腹腔镜TME相比,腹腔镜辅助taTME治疗超低位直肠癌的手术时间更长,但其可促进患者恢复,提高患者生存质量,值得临床推广。Objective:To investigate the effect of laparoscopic total mesorectal excision (TME) and laparoscopic-assisted transanal total mesorectal excision (taTME) in the treatment of ultra-low rectal cancer.Method:A total of 60 patients with ultra-low rectal cancer admitted in our hospital from July 2018 to July 2020 were selected as the subjects of this study,and they were randomly divided into two groups,with 30 patients in each group.The general group was treated with laparoscopic TME,and the experimental group was treated with laparoscopic-assisted taTME.The treatment effects were compared between the two groups.Result:The operation time of the experimental group was longer than that of the general group,but the exhaust time,hospital stay time,intraoperative blood loss were better than those of the general group,the differences were statistically significant (P<0.05).After treatment,PAC-QOL score of the experimental group was significantly better than that of the general group,and the difference was statistically significant (P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups (P>0.05).There were no significant differences in 1-year survival rate,local recurrence rate and distant metastasis rate between the two groups (P>0.05).Conclusion:Compared with laparoscopic TME,laparoscopic-assisted taTME takes longer operation time in the treatment of ultra-low rectal cancer,but it can promote patients’ recovery and improve patients’ quality of life,which is worthy of clinical promotion.
