腹腔灌洗液外泌体中miR-21、miR-320c、miR-1225-5p联合对胃癌腹膜转移的诊断价值

摘　　要：目的探讨腹腔灌洗液外泌体中miR-21、miR-320c、miR-1225-5p联合对胃癌腹膜转移的诊断价值。方法纳入2019年5月—2020年6月我院收治的胃癌患者298例,其中胃癌腹膜转移患者87例为研究组、胃癌非腹膜转移患者132例为转移组、胃癌预后良好患者79例为对照组。通过实时荧光定量PCR检测受试人员腹腔灌洗液外泌体中miR-21、miR-320c、miR-1225-5p的表达水平。用受试者工作特征曲线(ROC)的AUC分析腹腔灌洗液外泌体中miR-21、miR-320c、miR-1225-5p检测在胃癌腹膜转移中的诊断意义。结果研究组患者腹腔灌洗液外泌体中miR-21、miR-320c、miR-1225-5p的相对表达量均高于转移组和对照组患者,差异有统计学意义(P<0.05),研究组患者与对照组患者腹腔灌洗液外泌体中miR-21、miR-320c、miR-1225-5p的相对表达量差异无统计学意义(P>0.05)。腹腔灌洗液外泌体中miR-21、miR-320c、miR-1225-5p鉴别研究组患者与转移组和对照组患者的灵敏度分别为93%,91%,92%,特异度分别为93%,92%,91%。腹腔灌洗液外泌体中miR-21、miR-320c、miR-1225-5p联合对研究组的诊断的灵敏度为94.25%,特异度为90.99%,准确性为91.95%。结论腹腔灌洗液外泌体中miR-21、miR-320c、miR-1225-5p的表达量检测可作为诊断胃癌腹膜转移的潜在标志。Objective To investigate the diagnostic value of miR-21,miR-320c,miR-1225-5p in the exosomes of peritoneal lavage fluid in the diagnosis of gastric cancer peritoneal metastasis.Methods A total of 298 patients with gastric cancer admitted to our hospital from May 2019 to June 2020 were included.Among them,87 patients with gastric cancer and peritoneal metastasis were in the study group,132 patients with gastric cancer non-peritoneal metastasis were in the metastasis group,and 79 patients with good prognosis were in the gastric cancer group.Control group.Real-time fluorescent quantitative PCR was used to detect the expression levels of miR-21,miR-320c,and miR-1225-5p in the exosomes of the subjects'abdominal lavage fluid.The AUC of receiver operating characteristic curve(ROC)was used to analyze the diagnostic significance of miR-21,miR-320c,miR-1225-5p in peritoneal lavage fluid exosomes in gastric cancer peritoneal metastasis.Results The relative expressions of miR-21,mir-320c and mir-1225-5p in peritoneal lavage secretion of patients in the study group were significantly higher than those in the metastasis group and the control group(P<0.05).There was no significant difference in the relative expressions of miR-21,mir-320c and mir-1225-5p between patients in the study group and patients in the control group(P>0.05).The sensitivity and specificity of miR-21,mir-320c and mir-1225-5p in peritoneal lavage secretion were 93%,91%and 92%respectively.The sensitivity,specificity and accuracy of the combination of miR-21,mir-320c and mir-1225-5p in the secretion of peritoneal lavage fluid were 94.25%,90.99%and 91.95%.Conclusion The detection of the expression levels of miR-21,miR-320c and miR-1225-5p in the exosomes of peritoneal lavage fluid can be used as a potential marker for the diagnosis of peritoneal metastasis of gastric cancer.
