摘　　要：目的评价上海市口服抗帕金森病(PD)药品的可负担性。方法抽取上海市医药采购服务与监管信息系统2018年1月至12月中抗PD药品的价格,采用世界卫生组织/国际健康行动组织(WHO/HAI)标准调查法、药品致贫作用评价法(简称贫化法)和灾难性药品支出评价法(简称灾化法)评价上海市口服抗PD药品的可负担性。结果共调查14种、38个品规口服抗PD药品,涉及原研药和仿制药,仿制药和原研药的可负担性相差不大;自费情况下,不同口服抗PD药品全市致贫人口在上海市居民中的差异高达24倍;医保支付情况下,农村居民平均致贫率均为城镇居民的2倍。结论国家医疗保险大幅提高了抗PD药品的可负担性。建议国家积极推动新药研发进程,加快进口新药准入速度,做好现有药品价格的监督,必要时考虑纳入国家带量采购计划,以惠及更多帕金森病患者。Objective To evaluate the affordability of oral anti-Parkinson's disease(PD)drugs in Shanghai.Methods Based on the Shanghai Pharmaceutical Purchase Service and Supervision Information System,the prices of oral anti-PD drugs from January to December 2018 were selected,and the affordability of oral anti-PD durgs in Shanghai was evaluated by the World Health Organiza-tion/Health Action International(WHO/HAI)standard survey methodology,the assessment of disease-induced poverty and drug catas-trophic expenditure.Results A total of 14 kinds and 38 product specifications of oral anti-PD drugs were investigated,involving origi-nal patented drugs and generic drugs,but there was little difference in the affordability between generic drugs and original patented drugs.At the patient's own expense,the difference in the various oral anti-PD drugs-induced poverty-stricken people among Shang-hai residents was as high as 24 times.In case of the health insurance payment mode,the average poverty rate induced by oral anti-PD drugs in the rural residents was twice that in the urban residents.Conclusion The national health insurance policy has effectively improved the affordability of oral anti-PD drugs.It is suggested that the government should actively promote the research and develop-ment of new drugs,accelerate the access of imported new drugs,make efforts in the price supervision of the anti-PD drugs,and con-sider the procurement of drugs with target quantity when necessary,so as to benefit more PD patients.
