摘  要:目的构建医院院科两级绩效考核合理用药指标体系,提高医院合理用药管理水平。方法根据新医药卫生体制改革政策及国家对医院绩效考核管理的要求,结合合理用药管理现状,从医院合理用药发展战略和重点工作出发,运用价值树模型,分解、确定院级战略指标及科室关键指标间的逻辑关系,找出关键驱动流程,并根据各职能科室的工作内容与性质确定合理用药关键绩效考核指标。结果在运营管理部和合理用药持续质量改进工作组的推进下,构建了框架立体、科学全面、有效可行的院科两级关键绩效指标体系;2018年至2020年,处方点评数占处方总数的比例、抗菌药物使用强度、门诊患者基本药物处方占比、国家基本药物配备使用金额占比、国家组织药品集中采购中标药品使用占比等合理用药指标均得到了优化。结论分解、确定院级战略指标与科室关键指标间的逻辑关系,调整驱动战略,不断优化流程,对构建院科两级绩效考核合理用药指标体系具有一定借鉴意义。Objective To construct a performance appraisal index system for rational drug use at the hospital and department levels based on the value tree model,and to improve the management level of rational drug use in the hospital.Methods According to the new medical and health system reform policy and the national requirements for hospital performance appraisal management,combined with the current situation of the management of rational drug use and based on the development strategy and key work of rational drug use in the hospital,the value tree model was used to decompose and determine the logical relationship between hospital-level strategic indicators and department-level key indicators,find out the key driving process,and determine the key performance appraisal index of rational drug use according to the work content and nature of each functional department.Results Under the promotion of the opera-tion management department and the continuous quality improvement working group of rational drug use,a three-dimensional frame-work,scientific,comprehensive,effective and feasible key performance appraisal index system at the hospital and department levels was constructed.From 2018 to 2020,the indexes of rational drug use such as the proportion of reviewed prescriptions in the total number of prescriptions,antibiotics use density(AUD),the proportion of essential drug prescriptions of outpatients,the proportion of the amount of national essential drug allocation,and the proportion of the use of winning drugs in the drug centralized procurement organized by the state have been optimized.Conclusion Decomposing and determining the logical relationship between hospital-level strategic indicators and department-level key indicators,adjusting the driving strategy and constantly optimizing the process can be used as a reference for the construction of a performance appraisal index system of rational drug use at the hospital and department levels.
