激光治疗不同分型微静脉畸形的疗效及影响因素分析

摘　　要：目的:探讨激光治疗不同分型微静脉畸形的效果及影响因素分析。方法:收集2019年6月-2020年6月笔者科室就诊的100例微静脉畸形患儿(浅层血管型、深层血管型各50例)为研究对象,浅层血管型为治疗1组,深层血管型为治疗2组,给予595nm脉冲染料激光治疗。对比两组患儿的治疗效果。结果:治疗1组的治愈率、总有效率分别为14.0%、76.0%,治疗2组的治愈率、总有效率分别为6.0%、46.0%,治疗1组的治疗效果明显优于治疗2组(P<0.05)。随着治疗时间的增加,两组患儿的有效率逐渐增加,两组患儿治疗1个月、2个月后的疗效比较差异无统计学意义(P>0.05),治疗3个月后治疗1组患儿的治疗效果优于治疗2组,差异无统计学意义(P<0.05);微静脉畸形的治疗效果与治疗时间、疾病类型呈正相关,r值分别为0.531、0.612(P<0.05),与性别、年龄、体重指数、病灶部位无关,r值分别为0.134、0.067、0.094、0.016(P>0.05)。两组患儿治疗后不良反应(色素沉着、瘢痕、水疱、烧伤)发生率比较差异无统计学意义(P>0.05)。结论:595nm脉冲染料激光治疗微静脉畸形效果明显,尤其是浅层血管型的微静脉畸形有效率更高,术后不良反应的发生率低,治疗效果与分型、治疗时间具有相关性。Objective To investigate the effect of laser treatment on port wine stains of different types and the factors affecting the effect. Methods A total of 100 children with port wine stains who attended the Dermatology Clinic of our hospital from June 2019 to June 2020 were collected and divided into 2 groups: the treatment 1 group(superficial blood vessel type) and treatment 2 group(deep blood vessel type), 50 cases in each group. All were treated with 595 nm pulsed dye laser. Compare the therapeutic effects of 2 groups. Results The cure rate and total effective rate of the treatment 1 group were 14.0% and 76.0%,respectively. The cure rate and total effective rate of thetreatment 2 group were 6.0% and 46.0%, the treatment effect of the treatment 1 group was obviously better than that of the treatment 2 group(P<0.05). The effective rate of 2 groups gradually increases with the treatment time, and there is no statistical difference in the treatment effect between 2 groups after 1 month and 2 months’ treatment(P>0.05). After 3 months’ treatment, the treatment effect of patients in the treatment 1 group was better than that of the treatment 2 group(P<0.05). The treatment effect of port wine stains was positively correlated with treatment time and disease type, with r values of 0.531 and 0.612, respectively(P<0.05), independent of gender, age, body mass index and lesion location, with r values of 0.134, 0.067, 0.094 and 0.616(P>0.05). There was no significant difference in the incidence of complications(P>0.05). Conclusion 595 nm pulsed dye laser is effective in treating port wine stains,especially the superficial vascular type port wine stains.The incidence of postoperative complications is low. The therapeutic effect is related to the type and treatment time.
