正畸微型种植体对安氏Ⅱ类错牙合畸形患者美学疗效的影响

摘　　要：目的:分析正畸微型种植体对安氏Ⅱ类错牙合畸形患者的美学疗效。方法:选择2015年12月-2017年12月笔者医院收治的103例安氏Ⅱ类错牙合畸形患者为研究对象,采用随机数字列表法将其分为对照组(n=51)和研究组(n=52),对照组患者行传统正畸治疗,研究组患者行正畸微型种植体治疗,治疗18个月后,比较两组满意度、覆牙合(OB)及前牙覆盖(OJ)情况。结果:治疗后,研究组矫治满意度显著高于对照Z组(P<0.05);治疗前,两组OB、OJ比较差异无统计学意义(P>0.05);治疗后,两组OB、OJ较治疗前显著下降(P<0.05),研究组OB、OJ显著低于对照组(P<0.05);治疗前,两组IOTN-AC医生评价、IOTN-AC自我评价比较差异无统计学意义(P>0.05);治疗后,两组IOTN-AC医生评价、IOTN-AC自我评价较治疗前显著下降,差异具有统计学意义(P<0.05),研究组IOTN-AC医生评价、IOTN-AC自我评价显著低于对照组,差异具有统计学意义(P<0.05)。结论:正畸微型种植体治疗安氏Ⅱ类错牙合畸形患者疗效较好,能够改善患者侧貌,增加美学效果,患者满意度高,具有临床推广价值。Objective To investigate the aesthetic effect of orthodontic mini-implants in patients with Angle Class II malocclusion. Methods 103 patients with Angle Class II malocclusion in our hospital from December 2015 to December 2017 were enrolled, and randomly divided into two groups by random number table methods. Control group(n=51) received traditional orthodontic treatment, while study group(n=52) received orthodontic mini-implants. After 18 months of treatment,various indexes were compared, including treatment success rate, satisfaction, complication rate, overbite(OB), and overjet(OJ).Results The correction rate and satisfaction of study group were significantly higher than those of control group(P<0.05),the incidence of adverse reactions in study group was significantly lower than that of control group(P<0.05), OB and OJ had no difference between groups before treatment(P>0.05). After treatment, OB and OJ were decreased in both groups, and were lower in study group than in control group(P<0.05). The self-evaluation and doctor evaluation of Aesthetic Component of the Index of Orthodontic Treatment Need(IOTN-AC) had no significant difference between groups before treatment(P>0.05),which was improved in both groups after treatment(P<0.05), and was better in study group than in control group(P<0.05).Conclusion Application of orthodontic mini-implants can effectively improve aesthetic effects and reduce complications for patients with Angle Class Ⅱ malocclusion, which also obtains high satisfaction degrees.
