米非司酮联合血府逐瘀汤治疗子宫腺肌病的临床观察

摘　　要：目的研究米非司酮联合血府逐瘀汤治疗子宫腺肌病的临床效果。方法选取2020年1月-2021年6月南昌市洪都中医院妇科收治的60例子宫腺肌病患者,采用随机数字表分为观察组和对照组,各30例。对照组应用米非司酮治疗,观察组在对照组治疗基础上加血府逐瘀汤口服治疗,比较两组临床疗效、痛经情况(持续天数、疼痛程度)、经期症状积分(行经量、月经颜色)、性激素水平[促卵泡素(FSH)、促黄体生成素(LH)、雌二醇(E2)]、糖类抗原125(CA125)、子宫体积、不良反应。结果观察组治疗有效率高于对照组(P<0.05);治疗后,观察组痛经持续天数短于对照组,痛经程度、行经量及月经颜色评分均小于对照组(P<0.05);治疗后,两组FSH、LH、E2水平低于治疗前,且观察组低于对照组(P<0.05);观察组CA125低于对照组,子宫体积小于对照组(P<0.05);两组心、肝、肾功能及血、尿、粪常规等指标均无异常,无严重不良反应发生。结论米非司酮联合血府逐瘀汤治疗子宫腺肌病疗效确切,可改善患者经期症状及内分泌情况,降低血清肿瘤标志物水平,缩小子宫体积,且安全性高。Objective To study the clinical effect of mifepristone combined with Xuefu Zhuyu decoction in the treatment of adenomyosis.Methods A total of 60 patients with adenomyosis admitted to the Department of Gynecology,Nanchang Hongdu Hospital of Traditional Chinese Medicine from January 2020 to June 2021 were selected and divided into observation group and control group by random number table,with 30 cases in each group.The control group was treated with mifepristone,and the observation group was treated with Xuefu Zhuyu decoction on the basis of the treatment of the control group.The clinical efficacy,dysmenorrhea(duration,pain degree),menstrual symptom score(menstrual volume,menstrual color),the level of sex hormone[follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2)],carbohydrate antigen 125(CA125),uterine volume,and adverse reactions were compared between the two groups.Results The effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the duration of dysmenorrhea in the observation group was shorter than that in the control group,and the scores of dysmenorrhea,menstrual volume and menstrual color were lower than those in the control group(P<0.05).After treatment,the levels of FSH,LH and E2 in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).CA125 in the observation group was lower than that in the control group,and the uterine volume was smaller than that in the control group(P<0.05).There were no abnormalities in the heart,liver,kidney function,blood,urine,stool routine and other indicators in the two groups,and no serious adverse reactions occurred.Conclusion Mifepristone combined with Xuefu Zhuyu decoction has a definite effect in the treatment of adenomyosis,which can improve menstrual symptoms and endocrine conditions,reduce serum tumor marker levels,reduce uterine volume,and has high safety.
