前房注射过滤空气在原发性慢性闭角型青光眼治疗中的作用分析

摘　　要：目的探讨前房注射过滤空气在原发性慢性闭角型青光眼治疗中的作用。方法原发性慢性闭角型青光眼患者52例(86眼)随机分为对照组和观察组各43眼。对照组行复合式小梁切除术,观察组在行复合式小梁切除术基础上加前房注射过滤空气。术后随访6个月,比较分析两组疗效、并发症、视力及眼压变化情况。结果术后1日及术后3日观察组眼压值较对照组明显增高,术后3日观察组眼压已快速下降而恢复正常。对照组早期较低的眼压逐渐上升并趋于正常,观察组眼压则由高降低趋于稳定,眼压下降缓慢,眼压波动幅度极小。观察组术后浅前房、恶性青光眼和非计划再次手术发生数均明显少于对照组。观察组的住院时间和住院总费用较对照组明显减少。结论在复合式小梁切除术基础上加前房注射过滤空气治疗原发性慢性闭角型青光眼能减少恶性青光眼、术后浅前房及非计划再次手术的发生率,可减少住院时间,降低住院费用。Objective To investigate the effect of filtering air injected into anterior chamber in the treatment of primary chronic angle closure glaucoma.Method 52 patients(86 eyes)with primary chronic angle closure glaucoma were randomly divided into control group and observation group,43 eyes in each group.The control group was treated with compound trabeculectomy,and the observation group was treated with filtering air injection into an-terior chamber on the basis of compound trabeculectomy.After 6 months of follow-up,the curative effect,compli-cations,visual acuity and intraocular pressure of the two groups were compared and analyzed.Results The in-traocular pressure of the observation group was significantly higher than that of the control group at 1 and 3 days af-ter operation(P<0.05),and then decreased and returned to normal at 3 days after operation.In the early stage,the lower IOP in the control group gradually increased and tended to be normal,while in the observation group,the IOP decreased from high to stable,the IOP decreased slowly,and the fluctuation range of IOP was very small.The incidence of shallow anterior chamber,malignant glaucoma and unplanned reoperation in the observation group was significantly less than that in the control group(P<0.05).The length of stay and total cost of hospitalization in the observation group were significantly less than those in the control group(P<0.05).Conclution Combined trabe-culectomy combined with filtering air injection into anterior chamber in the treatment of primary chronic angle clo-sure glaucoma can reduce the incidence of malignant glaucoma,postoperative shallow anterior chamber and un-planned reoperation,reduce the length of hospital stay and hospitalization costs.
