摘　　要：目的评价三维动脉自旋标记(3D-ASL)对急性脑梗死患者功能恢复的预测价值。方法研究对象为155例急性脑梗死患者。根据动脉自旋标记(ASL)-磁共振扩散加权成像(DWI)图像,将患者分为低灌注组38例、高灌注组39例和等灌注组38例。评估3组患者治疗前及治疗后3个月功能恢复,应用功能独立性检查(FIM)评定量表及生活质量综合评分量表(GQOL-74)进行评定。结果治疗3个月后,3组患者FIM及GQOL-74评分较治疗前均升高(P<0.05),其中高灌注组变化程度最大(P<0.05)。采用Pearson相关分析显示,急性期脑梗死者高灌注与FIM和GQOL-74评分变化呈正相关(r=0.825,P=0.027;r=1.034,P=0.011)。结论3D-ASL可以早期评估急性脑梗死患者脑血流,灌注越好,预后越佳。Objective To evaluate the predictive value of three-dimensional arterial spin labeling(3D-ASL)for functional recovery in patients with acute cerebral infarction.Methods The subjects were 155 patients with acute cerebral infarction.According to ASL-diffusion weighted imaging(DWI)images,the patients were divided into low perfusion group(n=38),high perfusion group(n=39)and equal perfusion group(n=38).The functional independence measure(FIM)scale and generic quality of life inventory-74(GQOL-74)were evaluated before and 3 months after treatment.Results After 3 months of treatment,the scores of FIM and GQOL-74 in the three groups were higher than those before treatment(all P<0.05),and the change degree in the high perfusion group was the greatest(P<0.05).Pearson correlation analysis showed that there was a positive correlation between high perfusion and FIM and GPOL-74 scores in patients with acute cerebral infarction(r=0.825,P=0.027;r=1.034,P=0.011).Conclusion 3D-ASL could early evaluate cerebral blood flow in patients with acute cerebral infarction,the better the perfusion,the better the prognosis.
