旋覆代赭汤合六君子汤联合雷贝拉唑钠肠溶胶囊治疗反流性食管炎中虚气逆证临床观察

摘　　要：目的观察旋覆代赭汤合六君子汤联合雷贝拉唑钠肠溶胶囊治疗反流性食管炎(RE)中虚气逆证的临床疗效。方法将80例RE中虚气逆证患者按照随机数字表法分为2组。对照组40例予雷贝拉唑钠肠溶胶囊治疗,治疗组40例在对照组治疗基础上予旋覆代赭汤合六君子汤治疗。2组均治疗8周。比较2组治疗前后症状评分、镜下食管黏膜改变,治疗前及治疗4、8周反流性疾病问卷(RDQ)评分,并统计2组临床疗效、胃镜疗效,治疗结束6个月统计复发率。结果治疗组临床总有效率92.5%(37/40),对照组临床总有效率80.0%(32/40),治疗组临床疗效优于对照组(P<0.05)。治疗后2组反酸或泛吐清水、嗳气或反流、胃胀、食欲不振、乏力、便溏症状评分及总分均较本组治疗前降低(P<0.05),且治疗后治疗组均低于对照组(P<0.05)。治疗4、8周2组RDQ评分均较本组治疗前降低(P<0.05),治疗8周2组均较本组治疗4周降低(P<0.05),且治疗4、8周治疗组均低于对照组同期(P<0.05)。治疗后2组食管黏膜评分均较本组治疗前降低(P<0.05),且治疗后治疗组低于对照组(P<0.05)。治疗组胃镜总有效率92.5%(37/40),对照组胃镜总有效率85.0%(34/40),治疗组胃镜疗效优于对照组(P<0.05)。治疗组复发率低于对照组(P<0.05)。2组治疗期间未发生严重不良反应。结论旋覆代赭汤合六君子汤联合雷贝拉唑钠肠溶胶囊治疗RE中虚气逆证患者疗效显著,临床治愈率高、不良反应少、复发率低,值得临床推广。Objective To observe the clinical effect of Xuanfu Daizhe Decoction plus Liujunzi Decoction combined with rabeprazole sodium enteric-coated capsules for reflux esophagitis(RE)of deficiency of middle and qi.Methods Totallly 80 patients with RE(deficiency of middle and qi)were randomly divided into control group and treatment group,40 cases per group.Patients in control group were treated with rabeprazole sodium enteric-coated capsules,and patients in treatment group with Xuanfu Daizhe Decoction plus Liujunzi Decoction on the basis of control group.Treated for 8 weeks,TCM Symptom scores and changes of esophageal mucosal under gastroscopy before and after treatment were compared between groups,reflux disease questionnaires(RDQs)were performed before treatment,and on the 4th,8th postoperative week;the clinical efficacy,gastroscopic curative effect,and the 6th postoperative month recurrence rate were counted.Results The clinical efficacy rate in treatment group and control group was 92.5%(37/40)and 80.0%(32/40),respectively,the curative effect being better in treatment group than control group(P<0.05).After treatment,the TCM symptom scores including acid reflux,hydroptysis,belching,reflux,gastric distention,inappetence,fatigue,loose stool in groups were notably decreased(P<0.05),treatment group was lower than control group(P<0.05).Compared with before treatment,the 4th,8th postoperative week RDQ in groups was decreased(P<0.05),which decreased notably in treatment group when compared with control group(P<0.05);the 8th postoperative week RDQ in groups was obviously decreased in comparison with the 4th postoperative week RDQ(P<0.05);the esophageal mucosal score in groups was lower after treatment(P<0.05),improvement of which in treatment group was significantly better(P<0.05).The total effective rate of gastroscope in treatment group was better than that in control group[92.5%(37/40)vs 85.0%(34/40),(P<0.05),respectively];the recurrence rate was lower in treatment group than in control group(P<0.05);there was no serious a
