甘草人参汤联合西药治疗老年急性上消化道出血临床研究

摘　　要：目的:观察甘草人参汤联合西药治疗老年急性上消化道出血的临床疗效。方法:选取80例老年急性上消化道出血患者,按随机数字表法分为治疗组和对照组各40例。对照组给予卡络磺钠注射液联合法莫替丁注射液治疗,治疗组在对照组基础上给予甘草人参汤治疗,2组均治疗3 d。比较2组临床疗效、止血时间、血压稳定时间及腹痛缓解时间;比较2组治疗前后中医证候积分、血红蛋白水平及空腹胃液p H值。结果:治疗组总有效率为95.00%,高于对照组的80.00%(P<0.05)。治疗后,2组中医证候积分均较治疗前降低(P<0.05),治疗组中医证候积分低于对照组(P<0.05)。治疗组止血时间、血压稳定时间及腹痛缓解时间均短于对照组(P<0.05)。治疗后,2组血红蛋白水平、空腹胃液pH值均较治疗前升高(P<0.05);治疗组血红蛋白水平、空腹胃液pH值均高于对照组(P<0.05)。结论:甘草人参汤联合西药治疗老年急性上消化道出血,可有效缓解患者的临床症状,加速止血,维持血压稳定,提高血红蛋白水平及胃液pH值。Objective:To observe the clinical effect of Gancao Renshen tang combined with western medicine for acute upper gastrointestinal bleeding in senile patients.Methods:A total of 80 cases of senile patients with acute upper gastrointestinal bleeding were selected and divided into the treatment group and the control group according to random number table method,with 40 cases in each group.The control group was treated with carbosulfan sodium injection and famotidine injection,and the treatment group was additionally treated with Gancao Renshen tang based on the treatment of the control group.Both groups were treated for three days.The clinical effects,hemostasis time,the time to attine stable blood pressure and the remission time of abdominal pain were compared between the two groups.Before and after treatment,Chinese medicine syndrome scores,hemoglobin levels,and pH values of fasting gastric juice were compared between the two groups.Results:The total effective rate was 95.00%in the treatment group,higher than that of 80.00%in the control group(P<0.05).After treatment,Chinese medicien syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05),and the score in the treatment group was lower than that in the control group(P<0.05).Hemostasis time,the time to attain stable blood pressure and the remission time of abdominal pain in the treatment group were shorter than those in the control group(P<0.05).After treatment,hemoglobin levels and p H values of fasting gastric juice in the two groups were increased when compared with those before treatment(P<0.05);the above level and value in the treatment group were higher than those in the control group(P<0.05).Conclusion:For senile patients with acute upper gastrointestinal bleeding,the therapy of Gancao Renshen tang combined with western medicine can effectively relieve their clinical symptoms,accelerate hemostasis,stabilize the blood pressure and enhance hemoglobin level and pH value of gastric juice.
