超声透入化痰通络汤治疗缺血性脑卒中后肩痛Ⅰ期临床研究

摘　　要：目的:观察超声透入化痰通络汤治疗缺血性脑卒中后肩痛Ⅰ期的临床疗效。方法:选取100例缺血性脑卒中后肩痛Ⅰ期患者,按随机数字表法分为对照组和观察组,每组50例。观察组给予超声透入化痰通络汤治疗,对照组给予单纯超声波治疗,2组均治疗4周。比较2组治疗前后肩痛程度、上肢运动功能、日常生活活动能力、肩关节活动度、抑郁程度及睡眠质量;比较2组临床疗效。结果:治疗后,2组视觉模拟评分法(VAS)、汉密尔顿抑郁量表(HAMD)-17、匹兹堡睡眠质量指数(PSQI)评分均较治疗前降低(P<0.01),观察组VAS、HAMD-17、PSQI评分均低于对照组(P<0.01);2组上肢Fugl-Meyer运动功能评定量表(FMA)、改良Barthel指数(MBI)评分均较治疗前升高(P<0.01),观察组上肢FMA、MBI评分均高于对照组(P<0.01);2组肩关节活动度均较治疗前增大(P<0.01),观察组肩关节活动度大于对照组(P<0.01)。观察组愈显率为72.00%,高于对照组的48.98%(P<0.05)。结论:超声透入化痰通络汤治疗缺血性脑卒中后肩痛Ⅰ期疗效明显,可有效减轻患者的肩痛症状,提高其上肢运动功能、日常生活活动能力及睡眠质量,改善其情绪。Objective:To observe the clinical effect of phonophoresis of Huatan Tongluo tang on patients with stage-Ⅰshoulder pain after ischemic stroke.Methods:A total of 100 patients with stage-Ⅰshoulder pain after ischemic stroke were selected and divided into the control group and the observation group by random number table method,50 cases in each group.The observation group was treated with phonophoresis of Huatan Tongluo tang,and the control group was treated with simple ultrasonic therapy.Both groups were treated for four weeks.The degree of shoulder pain,upper limb motor function,activities of daily living,shoulder range of motion,depression and sleep quality were compared between the two groups before and after treatment.The clinical effect in the two groups was also compared.Results:After treatment,the scores of Visual Analogue Scale(VAS),Hamilton Depression Scale-17(HAMD-17)and Pittsburgh Sleep Quality Index(PSQI)in both groups were decreased when compared with those before treatment(P<0.01),and the above scores in the observation group were lower than those in the control group(P<0.01).The scores of Fugl-Meyer Assessment Scale(FMA)and Modified Barthel Index(MBI)of upper limbs in both groups were increased when compared with those before treatment(P<0.01),and the above two scores in the observation group were higher than those in the control group(P<0.01).The range of motion of shoulder in both groups was increased when respectively compared with that before treatment(P<0.01);the range of motion of shoulder in the observation group was increased when compared with that in the control group(P<0.01).The markedly effective rate in the observation group was 72.00%,higher than that of 48.98%in the control group(P<0.05).Conclusion:Phonophoresis of Huatan Tongluo tang has significant therapeutic effect on patients with stage-Ⅰshoulder pain after ischemic stroke,which can effectively reduce the symptoms of shoulder pain,improve their upper limb motor function,activities of daily living and sleep quality,and make th
