胃镜下应用可旋转重复开闭软组织夹联合圈套器治疗巨大胃石26例

摘　　要：目的:分析探讨胃镜下应用可旋转重复开闭软组织夹联合圈套器治疗巨大胃石的方法、疗效及安全性。方法:2017年1月~2019年12月于本院内镜室行纤维胃十二指肠镜检查患者中,发现巨大胃石患者26例,并于胃镜下应用可旋转重复开闭软组织夹联合圈套器治疗并观察治疗效果与安全性。结果:26例患者全部治疗成功,其中19例(73.1%)经1次镜下治疗胃石消失,5例(19.2%)经2次镜下治疗胃石消失,2例(7.7%)经3次镜下治疗胃石消失,所有患者均无粘膜损伤、出血、穿孔、感染、梗阻等并发症出现,临床症状均消失或明显缓解。结论:胃镜下应用可旋转重复开闭软组织夹联合圈套器治疗巨大胃石是经济有效安全的治疗方法,设备器械简单,治疗操作简便,安全有效,并发症风险低,患者痛苦少,治疗后恢复快,治疗费用低,便于临床广泛推广。Objective:To analyze the method,efficacy and safety of rotatable and repeated opening and closing soft tissue clip combined with snare in the treatment of giant gastrolith under gastroscope.Methods:From January 2017 to December 2019,26 patients with giant gastrolithiasis were found in the endoscopy room of our hospital.They were treated with rotatable and repeated opening and closing soft tissue clip combined with snare under gastroscope,and the treatment effect and safety were observed.Results:All 26 patients were treated successfully,of which 19 cases(73.1%)disappeared after one microscopic treatment,5 cases(19.2%)disappeared after two microscopic treatments,and 2 cases(7.7%)disappeared after three microscopic treatments.All patients had no complications such as mucosal injury,bleeding,perforation,infection and obstruction,and their clinical symptoms disappeared or significantly relieved.Conclusion:The application of rotatable and repeated opening and closing soft tissue clip combined with snare device under gastroscope in the treatment of giant gastrolith is an economic,effective and safe treatment method.The equipment and instruments are simple,the treatment operation is simple,safe and effective,the risk of complications is low,the patient's pain is less,the recovery is fast after treatment,and the treatment cost is low,which is convenient for wide clinical promotion.
