摘 要:目的:探讨产前三维超声诊断胎儿胼胝体发育不良的临床价值。方法:回顾性分析2018年7月~2020年7月就诊于沈阳经济技术开发区人民医院妇产科孕妇2400例,所有孕妇均行产前三维超声检查,观察胎儿的胼胝体并记录胎儿胼胝体的正常与否,对三维超声怀疑胎儿胼胝体发育不良者进行MRI检查,与MRI相比较,计算三维超声诊断胎儿胼胝体发育异常的准确性及诊断胎儿胼胝体发育不良的诊断效能。结果:产前三维超声诊断胎儿胼胝体发育不良21例,其中完全性胝体发育不良7例,不完全性胝体发育不良14例。MRI诊断胎儿完全性胼胝体缺失7例,胎儿不完全性胼胝体缺失12例。结论:产前三维超声能很好地观察胎儿胼胝体,能对胎儿胼胝体发育不良做出准确的诊断,具有较高的诊断效能,与MRI的诊断效能相一致,可作为产前筛查胎儿胼胝体发育不良的重要方法,是临床医师随诊及诊断胎儿胼胝体发育异常的首选方法,值得在临床上推广、应用。Objective:To explore the clinical value of prenatal three-dimensional ultrasound in the diagnosis of fetal corpus callosum dysplasia.Methods:2400 pregnant women in the Department of Obstetrics and Gynecology of the People’s Hospital of Shenyang Economic and Technological Development Zone from July 2018 to July 2020 were retrospectively analyzed.All pregnant women underwent prenatal three-dimensional ultrasound,observed the fetal corpus callosum and recorded whether the fetal corpus callosum was normal or not.MRI was performed on those who were suspected of fetal corpus callosum dysplasia by three-dimensional ultrasound,which was compared with MRI,The accuracy of three-dimensional ultrasound in the diagnosis of fetal corpus callosum dysplasia and the diagnostic efficiency in the diagnosis of fetal corpus callosum dysplasia were calculated.Results:21 cases of fetal dysplasia of corpus callosum were diagnosed by prenatal three-dimensional ultrasound,including 7 cases of complete dysplasia of corpus callosum and 14 cases of incomplete dysplasia of corpus callosum.MRI diagnosed 7 cases of fetal complete loss of corpus callosum and 12 cases of fetal incomplete loss of corpus callosum.Conclusions:Prenatal three-dimensional ultrasound can well observe the fetal corpus callosum and make an accurate diagnosis of fetal corpus callosum dysplasia.It has high diagnostic efficiency,which is consistent with the diagnostic efficiency of MRI.It can be used as an important method for prenatal screening of fetal corpus callosum dysplasia.It is the first choice for clinicians to follow-up and diagnose fetal corpus callosum dysplasia,it is worth popularizing and applying in clinic.
