改良穹窿结膜小切口术治疗小儿水平斜视恢复眼位和视功能的研究

摘　　要：目的研究改良穹窿结膜小切口术治疗小儿水平斜视后对眼位和视功能恢复的影响研究。方法方便选取2017年7月-2020年7月该院眼科收治的水平斜视患儿120例作为研究对象,按照入院编号采用信封均等法分为研究组和对照组,各60例。对照组采用角膜缘梯形结膜切口术治疗,研究组采用改良穹窿结膜小切口术治疗;比较两组治疗后眼位恢复、泪膜功能以及并发症率。结果治疗后,研究组治疗总有效率为93.33%显著高于对照组的76.67%,差异有统计学意义(χ^(2)=6.536,P=0.011);研究组并发症总发生率为6.67%显著低于对照组的25.00%,差异有统计学意义(χ^(2)=7.566,P=0.006)。治疗后,两组干眼症评分、CFS评分显著降低,且研究组低于对照组;BUT、SIt升高,且研究组高于对照组,差异有统计学意义(P<0.05)。结论改良穹窿结膜小切口术治疗小儿水平斜视疗效显著,改善泪膜功能,减少干眼的发生,术后并发症发过率低,安全可靠,值得推广。Objective To study the effect of modified fornix conjunctival incision on the recovery of eye position and visual function after the treatment of children with horizontal strabismus.Methods 120 children with horizontal strabismus treated in the ophthalmology department of the hospital from July 2017 to July 2020 were selected conveniently as the research object.According to the admission number,they were divided into the study group and the control group by envelope equalization method,with 60 cases in each group.The control group was treated with a limbal trapezoidal conjunctival incision,and the study group was treated with a modified fornix conjunctival small incision;the eye position recovery,tear film function and complication rate were compared between the two groups after treatment.Results After treatment,the total effective rate of 93.33%in the study group was significantly higher than that in the control group 76.67%,and the difference was statistically significant(χ^(2)=6.536,P=0.011).The total incidence of complications in the study group was 6.67%significantly lower than 25.00%in the control group,and the difference was statistically significant(χ^(2)=7.566,P=0.006).After treatment,the dry eye scores and CFS scores of the two groups were significantly reduced,and the study group was lower than the control group;BUT and SIt increased,and the study group was higher than the control group,and the difference was statistically significant(P<0.05).Conclusion The modified fornix conjunctival small incision has a significant effect in the treatment of children with horizontal strabismus,improves the function of the tear film,reduces the occurrence of dry eye,has a low postoperative complication rate,was safe and reliable,and was worthy of promotion.
