宫颈癌前病变HPV感染与宫颈液基细胞早期筛查的临床价值分析

摘　　要：目的探究宫颈癌前病变HPV感染与宫颈液基细胞早期筛查的临床价值。方法选取2017年12月至2020年3月于本院就诊的100例疑似宫颈癌前病变患者作为研究对象,患者均行高危型HPV检查与宫颈液基细胞检查。比较两种检查方式HPV阳性率、宫颈液基细胞异常率,阳性患者宫颈组织活检病理结果,宫颈细胞学阳性与宫颈组织活检病理结果。结果100例疑似宫颈癌前病变患者中,高危HPV感染的阳性19例,阳性率为19.00%,其中HPV16、HPV18占比较高,为42.10%、36.83%;异常细胞涂片11例,异常率为11.00%,其中占比前三的为ASC(45.45%)、LSIL(18.18%)、HSIL(18.18%);HPV16在病理学检查炎症中阳性率较高,为75%,HPV18在CNⅡ、CNⅢ中阳性率较高,分别为42.86%、28.57%;11例异常细胞涂片患者中,ASC在炎症中阳性率较高,为60.00%。结论宫颈癌前病变HPV感染与宫颈液基细胞早期筛查可显著提高宫颈癌患者病变筛查率,值得临床推广应用。Objective To explore the clinical value of HPV infection in cervical precancerous lesions and early screening of cervical liquid-based cells.Methods 100 patients with suspected cervical precancerous lesions who were treated in our hospital from December 2017 to March 2020 were selected as the research subjects.All patients underwent high-risk HPV examination and cervical liquid-based cell examination.The positive rate of HPV,the abnormal rate of cervical liquid-based cells,the pathological results of cervical biopsy in positive patients,the positive cervical cytology and the pathological results of cervical tissue biopsy were compared between two methods.Results Among 100 patients with suspected cervical precancerous lesions,19 cases were positive for high-risk HPV infection,with positive rate of 19.00%,of which HPV16 and HPV18 accounted for relatively high proportion of 42.10%and 36.83%;11 cases of abnormal cell smears,with abnormal rate of 11.00%,of which the top three were ASC(45.45%),LSIL(18.18%)and HSIL(18.18%);HPV16 has a higher positive rate in pathological examination was 75%,and HPV18 has higher positive rate in CNⅡand CNⅢ,42.86%and 28.57%respectively;among 11 patients with abnormal cell smears,the positive rate of ASC in inflammation was higher,which was 60.00%.Conclusion HPV infection of cervical precancerous lesions and early screening of cervical liquid-based cells can significantly improve the screening rate of cervical cancer patients,and it is worthy of clinical application.
