单纯性泪腺脱垂的手术效果及病理分析

摘　　要：目的探讨单纯性泪腺脱垂的手术效果及组织病理学变化特征。方法回顾性分析徐州市第一人民医院2016年8月至2020年11月单纯性泪腺脱垂手术28例(56眼)的临床资料。采用切开法双眼重睑成形术设计手术切口,对切除的部分泪腺进行组织病理学分析。比较术前及术后1个月的泪液分泌试验(SchirmerⅠ试验)、泪膜破裂时间(BUT),观察有无干眼的发生。术后随访4~48个月。结果术后脱垂的泪腺全部复位,双眼睑重睑形态自然,外观改善明显,无复发,术前及术后1个月SchirmerⅠ试验、BUT差异无统计学意义(P>0.05),无干眼发生。脱垂泪腺病理学改变为:泪腺内炎细胞浸润及纤维增生,呈慢性纤维硬化性泪腺炎改变。结论经重睑切口进行脱垂泪腺复位手术效果良好,可实现患者外观和功能的双重恢复。Objective To investigate the efficacy of surgery for simple lacrimal gland prolapse and it’s histopathological characteristics.Methods This was a retrospective case series study.The clinical data of 56 eyes of 28 patients with simple lacrimal gland prolapse who received surgical treatment from Aug.2016 to Nov.2020 in the First People’s Hospital of Xuzhou were collected.All cases underwent double-eyelid blepharoplasty.The histopathological changes of resected lacrimal gland were analyzed.The SchirmerⅠtest and break up time(BUT)were compared before and 1 month after operation.The occurrence of dry eye was observed.The follow-up time was 4 to 48 months.Results The eyelids of all cases were remodeled and lacrimal gland prolapse was repositioned.the double eyelid morphology of the patients was good,and the appearance was significantly improved.The SchirmerⅠtest and BUT were normal before and after operation.The difference was not statistically significant(P>0.05).No one suffered from dry eye syndrome after operation.The pathological changes of the prolapsed lacrimal gland included infiltration of inflammatory cells and hyperplasia of fibers in lacrimal gland,showing chronic and fibrous sclerotic inflammation of lacrimal gland.Conclusion Double-eyelid blepharoplasty combined with lacrimal gland reposition surgery is a method with good efficacy for the treatment of lacrimal gland prolapse to achieve both functional efficacy and aesthetic efficacy.
