摘　　要：目的评价改良Ahmed引流阀植入术治疗外伤性青光眼的效果。方法回顾性分析开封市中心医院2019年1月至2020年12月外伤性青光眼126例(126眼)的临床资料。患者分为两组,观察组行改良Ahmed青光眼引流阀植入术,对照组行小梁切除术。术后随访1年,观察两组的眼压、视力及并发症,分析视力恢复的影响因素。结果手术后眼压:观察组为(12.38±2.31)mmHg,低于对照组的(17.98±2.39)mmHg(t=-13.373,P<0.001);观察组视力优于对照组(t=2.488,P=0.014)。观察组术后并发症少于对照组(χ^(2)=21.421,P=0.017)。术后视力恢复的独立危险因素是手术方式、病程、年龄及术后眼压。结论治疗外伤性青光眼改良Ahmed引流阀植入术的效果优于小梁切除术。Objective To evaluate the efficacy of modified Ahmed drainage valve implantation for the treatment of traumatic glaucoma.Methods The clinical data of 126 eyes of 126 patients with traumatic glaucoma in Kaifeng Central Hospital from Jan.2019 to Dec.2020 were retrospectively analyzed.The patients were divided into two groups.The observation group received modified Ahmed glaucoma drainage valve implantation,and the control group received trabeculectomy.The postoperative follow-up time was 1 year.The intraocular pressure,visual acuity and complication of the two groups were observed.The risk factors of postoperative vision recovery were analyzed.Results After operation,the intraocular pressure was(12.38±2.31)mmHg in the observation group,which was lower than the(17.98±2.39)mmHg in the control group(t=-13.373,P<0.001);the postoperative visual acuity of the observation group was better than that of the control group(t=2.488,P=0.014).The complications of the observation group were less than those of the control group(χ^(2)=21.421,P=0.017).The independent risk factors for postoperative vision recovery were operation method,course of disease,age,and postoperative intraocular pressure.Conclusion The modified Ahmed drainage valve implantation is more effective than trabeculectomy for the treatment of traumatic glaucoma.
