摘　　要：目的评价前段玻璃体切除术及二期白内障手术治疗伴白内障前房极浅的急性闭角型青光眼的效果。方法回顾性分析上海同济医院2018年9月至2019年7月28例(35眼)原发性急性闭角型青光眼伴白内障患者。所有患者行前段玻璃体切除术,术后眼压稳定后行白内障摘出手术。的临床资料。术后随访1年,观察手术效果。结果术后1年,眼压从术前的(54.47±5.01)mmHg降至(17.70±2.35)mmHg(1mmHg=0.133 kPa)(t=34.519,P<0.001)。前房深度由(0.47±0.34)mm加深至(2.79±0.47)mm(t=0.890,P<0.001)。所有患者术后视力均有改善,均未发生术中或术后并发症。结论前段玻璃体切除术及二期白内障手术治疗伴白内障前房极浅的急性闭角型青光眼效果良好。Objective To evaluate the efficacy of anterior vitrectomy and second stage phacoemulsification combined with intraocular lens implantation for the treatment of acute angle-closure glaucoma with extremely shallow anterior chamber and cataract.Methods The clinical data of 35 eyes of 28 patients with acute angle-closure glaucoma and cataract from Sep.2018 to Jul.2019 in Shanghai Tongji Hospital were analyzed retrospectively.All cases with extremely shallow anterior chamber underwent anterior vitrectomy and second stage phacoemulsification after IOP stabilization.The therapeutic efficacy was observed during 1 year of follow-up.Results At 1 year after surgery,the intraocular pressure decreased significantly from(54.47±5.01)mmHg to(17.70±2.35)mmHg(1 mmHg=0.133 kPa)(t=34.519,P<0.001),and the anterior chamber depth increased significantly from(0.47±0.34)mm to(2.79±0.47)mm(t=0.890,P<0.001).All patients showed significant improvement in BCVA postoperatively and no intra-or post-operative complications occurred.Conclusion Anterior vitrectomy and second stage phacoemulsification combined with intraocular lens implantation is safe and effective for acute angle-closure glaucoma with cataracts and extremely shallow anterior chamber.
