ICL V4c植入术用于浅前房及小角膜的高度近视患者中的临床效果

摘　　要：目的观察浅前房及小角膜的高度近视患者行有晶状体眼后房型人工晶体(ICL)V4c植入术的临床效果。方法回顾性选取2019年8月至2020年12月在我院就诊后行ICL V4c植入术的浅前房及小角膜的高度近视患者共35例(67眼)。根据术前角膜水平直径(WTW)及前房深度(ACD)将其分为A、B、C组。比较三组患者的视力情况(UCVA、BCVA)、等效球镜(SE)、安全性指数、有效性指数、眼压、ACD、拱高、并发症发生情况。结果术后3个月,三组的UCVA、BCVA均较术前明显提高,SE较术前降低,差异具有统计学意义(P<0.05)。术后3个月,A、B、C组的SE在±1.00 D的比例分别为100.00%、96.00%、93.75%,安全性指数分别为(1.12±0.14)、(1.25±0.21)、(1.19±0.21),有效性指数分别为(1.12±0.14)、(1.23±0.19)、(1.17±0.23)。术前及术后3个月,三组的眼压比较,差异无统计学意义(P>0.05);术后3个月,A组与C组的眼压比较,差异具有统计学意义(P<0.05)。三组术后3个月的ACD与术前比较,差异具有统计学意义(P<0.05);术后3个月,A组与C组的ACD比较,差异具有统计学意义(P<0.05)。术后3个月,A、B、C组的拱高分别为170(145,285)、250(170,385)、300(230,447)μm(范围130~969μm);A、B组及A、C组的拱高比较,差异均无统计学意义(P>0.05)。所有患者术后均未出现严重并发症。结论浅前房(ACD<2.8 mm)及小角膜(WTW<10.8 mm)的高度近视患者行ICL V4c植入术具有良好的安全性及有效性,该术式能够有效改善患者术后视力和屈光状态。Objective To observe the clinical effect of implantable collamer lens(ICL)V4c implantation in high myopia patients with shallow anterior chamber and small cornea.Methods A total of 35 high myopia patients(67 eyes)with shallow anterior chamber and small cornea who diagnosed and underwent ICL V4c implantation in our hospital from August 2019 to December 2020 were selected retrospectively.The Patients were divided into groups A,B and C according to preoperative white-to-white(WTW)and anterior chamber depth(ACD).The visual acuity(UCVA,BCVA),spherical equivalent(SE),safety index,effectiveness index,intraocular pressure,ACD,arch height and complications of three groups were compared.Results Three months after operation,UCVA and BCVA of the three groups were significantly higher than those before operation,and SE was lower than that before operation,the differences were statistically significant(P<0.05).Three months after operation,the proportion of SE in±1.00 D in the groups A,B,C was 100.00%,96.00%and 93.75%respectively,the safety indexe was(1.12±0.14),(1.25±0.21),(1.19±0.21)respectively,and the effectiveness indexe was(1.12±0.14),(1.23±0.19),(1.17±0.23)respectively.There was no significant difference in intraocular pressure among the three groups before openation and on three months after operation(P>0.05);three months after operation,there was significant difference in intraocular pressure between the group A and the group C(P<0.05).There was significant difference in ACD among the three groups on 3 months after operation and before operation(P<0.05);three months after operation,there was significant difference in ACD between the group A and the group C(P<0.05).Three months after operation,the arch heights of groups A,B and C were 170(145,285),250(170,385)and 300(230,447)μm(range 130-969μm)respectively,and there was no significant difference in arch height between groups A and B,between groups A and C(P>0.05).No serious complications occurred in all patients.Conclusion ICL V4c implantation applied in high m
