剖宫产术联合子宫肌瘤剔除术治疗妊娠合并子宫肌瘤的疗效

摘　　要：目的分析剖宫产术联合子宫肌瘤剔除术治疗妊娠合并子宫肌瘤的疗效。方法100例妊娠合并子宫肌瘤患者,采用随机数字表法分为对照组及观察组,每组50例。对照组患者给予单纯剖宫产手术,术后择期实施子宫肌瘤剔除术治疗;观察组实施剖宫产术联合子宫肌瘤剔除术治疗。比较两组手术时间、住院时间、恶露干净时间、产后抗菌药物使用时间、产后出血量、并发症发生率。结果观察组手术时间(55.21±10.78)min长于对照组的(41.21±8.40)min,住院时间(5.21±1.23)d、恶露干净时间(6.15±0.45)d短于对照组的(8.21±2.21)、(9.27±2.24)d,产后出血量(251.15±6.22)ml少于对照组的(278.27±12.24)ml,差异具有统计学意义(P<0.05);两组产后抗菌药物使用时间比较,差异无统计学意义(P>0.05)。观察组并发症发生率为2.00%,低于对照组的16.00%,差异具有统计学意义(P<0.05)。结论剖宫产术联合子宫肌瘤剔除术治疗妊娠合并子宫肌瘤的疗效确切,虽然手术时间延长,但并发症更少,产后出血更少。Objective To analyze the efficacy of cesarean section combined with myomectomy on pregnancy complicated with hysteromyoma.Methods A total of 100 cases of pregnancy with hysteromyoma were randomly divided into control group and observation group according to random numerical table,with 50 cases in each group.Patients in the control group received cesarean section surgery only,and elective treatment of uterine myomectomy after the operation;the observation group recevied cesarean section and myomectomy for treatment.Both groups were compared in terms of operation time,hospitalization time,lochia cleaning time,postpartum antibiotics use time,amount of postpartum hemorrhage,and complication rate.Results The operation time(55.21±10.78)min of the observation group was longer than(41.21±8.40)min of the control group;the hospitalization time(5.21±1.23)d and lochia cleaning time(6.15±0.45)d of the observation group were shorter than(8.21±2.21)and(9.27±2.24)d of the control group;the amount of postpartum hemorrhage(251.15±6.22)ml was less than(278.27±12.24)ml of the control group;all the differences were statistically significant(P<0.05).There was no statistically significant difference between the two groups of postpartum antibiotics use time(P>0.05).The complication rate in the observation group was 2.00%,which was lower than 16.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion Cesarean section combined with myomectomy has definite efficacy on pregnancy complicated with hysteromyoma.Although the operation time is prolonged,there are fewer complications and less postpartum hemorrhage.
