单向倒刺线在胃癌根治术中的应用研究

摘　　要：目的探讨单向倒刺线在胃癌根治术中应用的安全性和有效性。方法选取2019年1月至2019年12月间昆明医科大学第一附属医院收治的行腔镜辅助下胃癌根治术+BillrothⅠ式并用单向倒刺线连续缝合胃肠吻合口的45例患者为倒刺线组,另选取同期行常规可吸收线间断缝合胃肠吻合口的45例患者为对照组,比较两组患者的术中出血量、吻合口狭窄情况、吻合口出血情况、吻合口瘘、术后住院时间和总住院时间。结果倒刺线组患者缝合修补胃肠吻合口手术时间较对照组短,差异有统计学意义(P<0.05)。但倒刺线组未明显增加患者手术出血、术后住院时间、手术时间和住院费用,差异无统计学意义(P>0.05)。两组患者吻合口瘘、吻合口出血和吻合口狭窄等相关并发症发生率比较,差异无统计学意义(P>0.05)。结论单向倒刺线连续缝合应用于腔镜辅助下胃癌根治术吻合口缝合,可缩短手术时间,安全有效。Objective To study the safety and effectiveness of unidirectional barbed suture during radical gastrectomy.Methods Forty-five patients who underwent endoscopic-assisted radical gastrectomy plus continuous suture for BillrothⅠanastomosis using unidirectional barbed suture were selected and included in a unidirectional barbed group and another 45 patients who underwent the same surgery using conventional absorbable interrupted sutures for gastrointestinal anastomosis were selected and included in a control group at The First Affiliated Hospital of Kunming Medical University from January 2019 to December 2019.Intraoperative blood loss,anastomotic stenosis,anastomotic bleeding,anastomotic leak,postoperative length of hospital stay and the overall length of hospital stay were compared between the two groups.Results The duration of operation for anastomotic suture repair was shorter in the unidirectional barbed group than in the control group(P<0.05).It was noteworthy that no obvious increase was discovered in intraoperative blood loss,postoperative length of hospital stay,the duration of operation and hospital expenditure.There was no statistically significant difference in complications like anastomotic leak,anastomotic bleeding and anastomotic stenosis between the two groups(P>0.05).Conclusion Continuous suture with unidirectional barbed suture can reduce the duration of operation with high safety and efficacy in endoscopic-assisted radical gastrectomy.
