双歧杆菌四联活菌片对Hp阳性十二指肠球部溃疡患者胃肠激素水平及机体免疫功能的影响

摘　　要：目的:观察幽门螺杆菌(Helicobacter pylori,HP)阳性十二指肠球部溃疡(Duodenal ulcer,DU)患者应用双歧杆菌四联活菌片的效果。方法:选取我科2018年8月至2020年10月期间103例HP阳性DU患者,以数字表法分对照组和观察组。对照组51例给予阿莫西林+克拉霉素+雷贝拉唑+枸橼酸铋钾治疗,观察组52例增加双歧杆菌四联活菌片,对比两组患者各项生理指标。结果:治疗14d后,观察组溃疡愈合率90.38%高于对照组74.50%(P<0.05),观察组HP根除率94.23%高于对照组80.39%(P<0.05)。观察组胃蛋白酶原Ⅱ、胃泌素17及胃蛋白酶原I水平均低于对照组(P<0.05)。观察组免疫球蛋白M、免疫球蛋白G及免疫球蛋白A水平高于对照组(P<0.05)。结论:铋剂四联药物联合双歧杆菌四联活菌片治疗HP阳性十二指肠球部溃疡能有效改善胃肠激素水平,提高免疫水平,促进溃疡愈合,提升HP根除效果。Objective:To observe the effect of Bifidobacterium quadruple viable tablets in patients with Helicobacter pylori(HP)-positive duodenal ulcer(DU).Methods:103 patients with HP positive DU in our department from August 2018 to October 2020 were divided into groups by digital table method.51 patients in the control group were treated with amoxicillin+clarithromycin+rabeprazole+bismuth potassium citrate.52 patients in the observation group were added with Bifidobacterium tetralogy viable tablets.The physiological indexes of the two groups were compared.Results:After 14 days of treatment,the ulcer healing rate in the observation group(90.38%)was higher than that in the control group(74.50%)(P<0.05),and the HP eradication rate in the observation group(94.23%)was higher than that in the control group(80.39%)(P<0.05).The levels of pepsinogenⅡ,gastrin 17 and pepsinogen I in the observation group were lower than those in the control group(P<0.05).The levels of immunoglobulin M,immunoglobulin G and immunoglobulin A in the observation group were higher than those in the control group(P<0.05).Conclusion:Bismuth tetralogy combined with Bifidobacterium tetralogy viable tablets in the treatment of HP positive duodenal ulcer can effectively improve the level of gastrointestinal hormones,improve the immune level,promote ulcer healing and enhance the effect of HP eradication.
