中性粒细胞与淋巴细胞比值预测食管癌术后吻合口瘘的临床价值

摘　　要：目的:探讨中性粒细胞与淋巴细胞比值(NLR)早期预测食管癌术后吻合口瘘的价值。方法:选取食管癌手术患者34例,其中试验组吻合口瘘患者17例,对照组未发生吻合口瘘患者17例。术后检测外周静脉血血常规,记录白细胞计数,计算NLR。比较两组患者NRL差异。结果:两组术后第1、3、6天白细胞(WBC)计数比较,差异均无统计学意义(P>0.05)。试验组术后第3天及第6天NLR明显高于对照组,差异均有统计学意义(P<0.05)。术后第3天NLR预测食管癌术后吻合口瘘的灵敏度为76.5%,特异度为75.3%,AUC为0.772,临界值为12.93。术后第6天NLR预测食管癌术后吻合口瘘的灵敏度为76.5%,特异度为76.5%,AUC为0.775,临界值为8.24。结论:食管癌术后第3天及第6天NLR可以较准确地预测吻合口瘘发生。Objective:To investigate the value of neutrophil to lymphocyte ratio(NLR)in early prediction of anastomotic leakage after esophageal cancer surgery.Methods:34 patients with esophageal cancer surgery were selected,including 17 patients with anastomotic leakage in the experimental group and 17 patients without anastomotic leakage in the control group.After the operation,the peripheral venous blood was tested,the white blood cell count was recorded,and the NLR was calculated.The NRL difference between the two groups of patients was compared.Results:There was no statistically significant difference in WBC counts between the two groups at 1,3,and 6 days after surgery(P>0.05).The NLR of the experimental group was significantly higher than the control group on the 3rd and 6th day after operation,the differences were statistically significant(P<0.05).On the 3rd day after operation,the sensitivity,specificity,AUC and critical value of NLR in predicting anastomotic leakage after esophageal cancer surgery were 76.5%,75.3%,0.772 and 12.93 respectively.On the 6th day after operation,the sensitivity,specificity and AUC of NLR in predicting anastomotic leakage after esophageal cancer surgery were 76.5%,76.5%,0.775 and 8.24 respectively.Conclusion:NLR can accurately predict the occurrence of anastomotic leakage on the 3rd and 6th day after esophageal cancer surgery.
