不同肥胖症诊断标准的肥胖与绝经后子宫内膜样腺癌患者病理指标的相关性分析

摘　　要：目的探讨不同肥胖诊断标准的肥胖与绝经后子宫内膜样腺癌患者病理结果的相关性。方法收集手术治疗的146例绝经后子宫内膜样腺癌患者的临床资料,分析不同肥胖诊断标准的肥胖与子宫内膜样腺癌患者临床病理学特征的相关性。结果单因素分析显示,按照中国成年人肥胖标准,肥胖患者腹水脱落细胞学阳性比例高于非肥胖患者(7.3%vs.1.0%)(P<0.05),但浸润深度、分化程度、脉管浸润、淋巴结转移和肿瘤最大直径等高危因素在两组患者间无统计学差异(P>0.05);而按照WHO制定的肥胖诊断标准,子宫内膜样腺癌病理因素在肥胖与非肥胖组间分布差异无统计学意义(P>0.05)。结论基于BMI制定的肥胖诊断标准,肥胖与绝经后患者子宫内膜样腺癌病理因素无明显相关性。Objective To explore the relationship between obesity and clinicopathological characteristics of post-menopausal patients with endometrioid endometrial carcinoma.MethodsClinical data of 146 post-menopausal patients with endometrioid endometrial carcinoma were collected and the correlation between obesity and clinicalpathological features were analyzed.Results The univariate analysis showed that, according to the "guidelines for prevention and control of overweight and obesity in Chinese adults",the rate of positive peritoneal cytology in obese patients was significantly higher than that in non-obese patients(7.3% vs.1.0%) and there were no obvious differences in the high risk factors including the depth of invasion, tumor grade, lymphovascular invasion, lymph node metastasis and maximum diameter of tumor between the obese and non-obese patients(P>0.05).According to the guidelines of WHO,there were no obvious differences in clinicopathological features between the obese and non-obese patients(P>0.05).Conclusion Taking the diagnostic criteria based on BMI in post-menopausal patients, there is no significant correlation between the clinicopathological features of endometrioid endometrial carcinoma and the obesity.
