摘 要:目的:检测肝细胞癌(hepatocellular carcinoma,HCC)患者环氧合酶-2(cyclooxygenase-2,COX-2)在癌组织及癌旁肝组织中的表达情况,并分析其临床病理意义。方法:选取南通市第三人民医院病理科存档的HCC患者石蜡样本344例,通过组织芯片结合免疫组织化学方法检测COX-2在癌组织及癌旁肝组织中的表达,统计学分析其表达与临床病理资料之间的相关性。结果:COX-2蛋白阳性表达定位于细胞质;COX-2在癌旁肝组织和HCC组织中阳性表达率分别为52.3%和43.9%,癌旁肝组织中COX-2蛋白表达高于癌组织(χ^(2)=4.896,P=0.027);COX-2在癌旁肝组织中的表达与患者乙肝表面抗原阳性(P=0.013)、有肝硬化(P=0.018)、微血管侵犯(P=0.037)及术后复发相关(P<0.001)。结论:COX-2在癌旁肝组织中高表达,且与术后复发有关,提示COX-2可能参与HCC的发生发展过程,检测癌旁肝组织中COX-2表达有助于预测HCC复发。Objective:To detect the expression of cyclooxygenase-2(COX-2)in hepatocellular carcinoma(HCC)and adjacent liver tissues,and analyze its clinicopathological significance.Methods:344 paraffin samples of HCC patients archived in the Department of Pathology,the Third People′s Hospital of Nantong were selected.The expression of COX-2 in cancer tissues and adjacent liver tissues was detected by tissue microarray and immunohistochemistry,and the correlation between COX-2 expression and clinicopathological data was statistically analyzed.Results:The positive expression of COX-2 protein was located in the cytoplasm.The positive expression rates of COX-2 in adjacent tissues and HCC tissues were 52.3%and 43.9%,respectively.The expression of COX-2 protein in adjacent tissues was higher than that in cancer tissues(χ^(2)=4.896,P=0.027).The expression of COX-2 in adjacent liver tissues was correlated with hepatitis B surface antigen infection(P=0.013),cirrhosis(P=0.018),microvascular invasion(P=0.037)and postoperative recurrence(P<0.001).Conclusion:High COX-2 expression may take part in the HCC genesis and development process.Detecting the expression of COX-2 may be helpful in prediction of early recurrence of HCC patients.
