改良闭口式印模法和生物功能性修复系统在低平牙槽嵴下颌总义齿修复中的临床效果研究

摘　　要：目的观察和评价下颌低平牙槽嵴的无牙颌患者采用改良闭口式印模法和生物功能性修复系统(BPS)制作总义齿的临床疗效。方法选取2016年3月至2019年3月到常州市第二人民医院口腔科就诊的41例存在严重下颌牙槽嵴吸收的无牙颌患者作为研究对象,采用随机数字表法分为观察组(21例)和对照组(20例)。观察组采用改良闭口式印模法进行总义齿修复,对照组采用BPS法进行总义齿修复。比较两组的主观满意度以及修复后3、6个月咀嚼效能。结果两组的主观满意度比较,差异无统计学意义(P>0.05)。修复后3、6个月,两组的组间和组内咀嚼效能比较,差异无统计学意义(P>0.05)。结论改良闭口式印模法和BPS法制作的总义齿均可取得良好的修复效果。改良闭口式印模法相对设备要求低,技术敏感低,步骤更少,对于下颌牙槽嵴重度吸收无牙患者,有着较好的临床疗效,可作为总义齿修复的常规印模方法。Objective To observe and evaluate the clinical effect of modified closed mouth impression method and biological functional system(BPS)for complete denture in edentulous patients with severe alveolar ridge absorption.Methods From March 2016 to March 2019,a total of 41 edentulous patients with severe mandibular alveolar ridge absorption in Department of Stomatology,Changzhou Second People′s Hospital were selected as research objects and divided into observation group(n=21)and control group(n=20)according to random number table method.The observation group performed complete denture repair using the modified closed mouth impression method,and the control groups performed complete denture repair using the BPS.The subjective satisfaction and masticatory efficiency at 3 and 6 months after repair were compared between the two groups.Results There was no statistically significant difference of subjective satisfaction between the two groups(P>0.05).At 3 and 6 months after repair,there were no statistically significant differences of masticatory efficiency between the two groups or in the group(P>0.05).Conclusion Both the modified closed mouth impression method and BPS method can achieve good restoration effect of complete denture.However,the improved closed mouth impression method has easy equipment requirements,lower technical sensitivity and the fewer steps.For patients with severe absorption of mandibular alveolar crest,it has good clinical efficacy and can be used as a conventional printing method for total denture repair.
