摘　　要：目的探讨自闭式回肠造口在低位直肠癌手术中的应用价值。方法选择2017年2月~2020年4月我院收治的104例行预防性造口的直肠癌患者作为观察对象,其中行回肠末端袢式造口患者69例,纳入袢式造口组,行自闭式回肠造口手术患者35例,纳入自闭造口组。收集两组患者手术时间、失血量、肠功能恢复时间、住院时间、住院费用等指标;比较两组术后近期及远期并发症发生情况。结果两组患者手术时间、失血量、肠功能恢复时间等资料比较差异无统计学意义(P>0.05),袢式造口组患者术后住院时间长于自闭式造口组,住院费用高于自闭式造口组(P<0.05)。两组患者近期及远期并发症比较差异均无统计学意义(均P>0.05)。结论相比于回肠袢式造口,自闭式回肠造口可避免二期回纳手术,缩短患者术后恢复时间,减少住院费用,有较高的临床应用价值。Objective To explore the application value of loop ileostomy in low rectal cancer surgery.Methods A total of 104 patients with rectal cancer who underwent preventive stoma and were admitted to the hospital from February 2017 to April 2020 were enrolled as the research objects,including 69 cases who underwent loop ileostomy of terminal ileum(loop ileostomy group)and 35 cases who underwent spontaneously closed ileostomy(spontaneously closed ileostomy group).The data such as operation time,blood loss,recovery time of intestinal function,hospitalization time and hospitalization cost were collected.The short-term and long-term complications after surgery were collected.Results There were no significant differences in operating time,blood loss and recovery time of intestinal function between the two groups(P>0.05).The postoperative hospitalization time in loop ileostomy group was longer than that in spontaneously closed ileostomy group,and hospitalization cost was more than that in spontaneously closed ileostomy group(P<0.05).There were no significant differences in short-term and long-term complications between the two groups(P>0.05).Conclusion Compared with loop ileostomy,spontaneously closed ileostomy can avoid second-stage temporary stoma-closure surgery,shorten postoperative recovery time,and reduce hospitalization cost.
