磁共振成像与多层螺旋CT诊断结肠癌致肠梗阻的临床价值分析

摘　　要：为探讨磁共振成像与多层螺旋CT在结肠癌致肠梗阻中的临床诊断价值,将结肠癌致肠梗阻患者100例随机分为磁共振成像组和多层螺旋CT组,每组50例,分析2组诊断方式的价值(检出准确率、误诊率、漏诊率)以及影像学特征(狭窄段、侵犯浆膜下或肠旁组织、狭窄段与正常肠管交界处改变、盲肠扩张、缺血性结肠炎)。结果显示,磁共振成像组诊断的检出准确率为100.00%,多层螺旋CT组诊断的检出准确率为90.00%,磁共振成像组的准确率显著高于多层螺旋CT组(P<0.05);2组误诊率比较差异无统计学意义(P>0.05);磁共振成像组漏诊率为零,多层螺旋CT组为8.00%,2组比较差异有统计学意义(P<0.05)。观察2组的影像学特征,磁共振成像组结肠癌致肠梗阻患者狭窄段、侵犯浆膜下或肠旁组织、狭窄段与正常肠管交界处改变、缺血性结肠炎的检出准确率均显著高于多层螺旋CT组(P<0.05)。结果表明,磁共振成像诊断结肠癌致肠梗阻检出准确率以及其他征象检出率明显高于多层螺旋CT诊断,为患者后期疾病的治疗提供了很大的数据支持,具有一定的临床价值。This study was to explore the clinical value of MRI and multi-slice spiral CT(mssCT)in diagnosis of intestinal obstruction(IO)induced by colonic cancer,randomly divided 100cases met above conditions into MRI group(50cases)and mssCT group(50cases);then,analysed the value of above two diagnosis patterns(including inspected-out accuracy rate,misdiagnostic rate,the rate of missed diagnosis),and image features such as stenosis segment,invaded tissue nearby bowel or of subserosa,the change between stenosis segment and normal bowel vessels,typhlectasis,and ischemic colitis.As results,in inspected-out rate MRI group was significantly higher than mssCT group(100.00%vs 90.00%,P<0.05);in misdiagnosis rate there was no statistical difference between the two groups(P>0.05);in the rate of missed diagnosis the difference between the two groups had statistical significance(0vs 8.00%,P<0.05);in the inspected-out accuracy rate of above-mentioned image features MRI group was all significantly higher than mssCT group(P<0.05).Results show that in inspected-out accuracy rate of IO induced by colonic colitis and other relevant feature of images MRI are all significantly higher than mssCT which will provide massive data support for further treatment,thus has certain clinical value.
