摘　　要：目的探讨脑室—腹腔分流术以及颅骨修补术在脑外伤中的应用价值。方法纳入2016年1月—2018年12月90例脑外伤患者根据手术分组。分期手术治疗组在脑室—腹腔分流术后2个月开展颅骨修补术治疗,同期手术治疗组开展同期脑室—腹腔分流术以及颅骨修补术。分析效果;残疾的发生率;手术前后患者格拉斯哥昏迷评分以及神经功能NHISS评分、四肢运动功能评分值;脑水肿、硬膜下积液、肺部感染等的发生率。结果同期手术治疗组效果、格拉斯哥昏迷评分以及神经功能NHISS评分、四肢运动功能评分值、残疾的发生率、脑水肿、硬膜下积液、肺部感染等总发生率与分期手术治疗组比较均有优势,差异有统计学意义(P <0.05)。结论脑外伤患者行同期脑室—腹腔分流术以及颅骨修补术效果确切,可有效减少并发症的发生,改善神经功能和昏迷,并改善四肢运动功能,降低致残率。Objective To explore the application value of ventriculoperitoneal shunt and cranioplasty in traumatic brain injury. Methods Ninety patients with brain trauma from January 2016 to December 2018 were divided into two groups according to operation. Two months after ventriculoperitoneal shunt operation, the treatment group underwent cranioplasty, while the treatment group underwent ventriculoperitoneal shunt and cranioplasty at the same time. The incidence of disability, Glasgow coma score,neurological NHISS score and limb motor function score before and after operation, brain edema, subdural effusion and pulmonary infection were analyzed. Results The effect of operation group, Glasgow coma score, NHISS score of nerve function, motor function score of limbs, incidence of disability, incidence of brain edema, subdural effusion and pulmonary infection in the same period were significantly better than those in the staging operation group(P<0.05).Conclusion Simultaneous ventriculoperitoneal shunt and cranioplasty are effective in reducing complications,improving neurological function and coma, improving limb motor function and reducing disability rate in patients with brain trauma.
