摘 要:目的回顾性统计分析恶性肿瘤患者血浆Ⅷ因子(FⅧ)、D-二聚体(D-D)、纤维蛋白(原)降解产物(FDP)、纤维蛋白原(Fbg)、抗凝血酶(AT)5项凝血指标的变化与肿瘤的关系。方法统计2019年5月至2020年3月期间共计2825例恶性肿瘤患者(1398例消化道肿瘤、1427例呼吸道肿瘤)、1901例良性疾病患者(917例为良性消化道疾病、984例为良性呼吸道疾病)和1073例体检健康者(健康对照组)所检测的FⅧ、D-D、FDP、Fbg、AT的水平。分别比较恶性肿瘤组(消化道肿瘤和呼吸道肿瘤)、良性疾病组(良性消化道和呼吸道疾病)与健康对照组之间5项凝血指标的差异。结果恶性肿瘤组、良性疾病组FⅧ、D-D、FDP、Fbg的水平均显著高于健康对照组,差异有统计学意义(P<0.01),而AT的水平则均明显低于健康对照组,差异有统计学意义(P<0.01);与呼吸道肿瘤组比较,消化道肿瘤组FⅧ、D-D、FDP、AT的水平显著升高,差异有统计学意义(P<0.01),Fbg则差异无统计学意义。消化道肿瘤组FⅧ、D-D、FDP、Fbg的水平显著高于消化道良性组,差异有统计学意义(P<0.01),AT则差异无统计学意义。呼吸道肿瘤组FⅧ、D-D、FDP、Fbg的水平显著低于呼吸道良性组,差异有统计学意义(P<0.01),AT的水平则显著高于呼吸道良性组,差异有统计学意义(P<0.01)。结论恶性肿瘤细胞通过多种途径与止血系统相互作用,使凝血与纤溶系统功能紊乱,血液处于易栓状态。本文通过回顾性分析明确了FⅧ、D-D、FDP、Fbg、AT的联合检测有利于了解恶性肿瘤患者的高凝与纤溶状态,对疗效评估及预后判断均具有十分重要的临床应用价值。Objective Retrospectively analyze the relationship between 5 blood coagulation indexes and tumors,including plasma tumor factorⅧ(FⅧ),D-dimer(DD),fibrinogen degradation products(FDP),fibrinogen(Fbg),antithrombin(AT).Methods Results of the 5 blood coagulation indexes were collected in 2,825 malignant tumor patients(1,398 gastrointestinal tumor and 1,427 respiratory tract tumor patients),1901 patients with benign diseases(917 cases of benign gastrointestinal disease and 984 cases of benign respiratory tract disease)and 1073 healthy people(normal control group)from May 2019 to March 2020.The coagulation indexes were compared between the malignant tumor group(digestive tract tumor group and respiratory tract tumor group),benign disease group(benign gastrointestinal group and benign respiratory group)and normal control group.Results The levels of FⅧ,DD,FDP,and Fbg in the malignant tumor group and benign disease group were significantly higher than the normal control group,the difference was statistically significant(P<0.01),while the levels of AT were significantly lower than the normal control group,the difference was statistically significant(P<0.01).Compared with the respiratory tract tumor group,the levels of FⅧ,DD,FDP,and AT in the digestive tract tumor group were significantly higher than those in the respiratory tract tumor group,the difference was statistically significant(P<0.01),but there was no statistical difference in Fbg.The levels of FⅧ,DD,FDP,and Fbg in the digestive tract tumor group were significantly higher than those of the digestive tract benign group,the difference was statistically significant(P<0.01),and AT had no statistical differences.The levels of FⅧ,D-D,FDP,and Fbg in the respiratory tract tumor group were significantly lower than those in the benign respiratory tract,the difference was statistically significant(P<0.01),and the levels of AT were significantly higher than those in the benign respiratory tract,the difference was statistically significant(P<0.01).Conclusion Maligna
