术中应用洛铂预防胃癌术后复发的临床研究

摘　　要：目的探索术中应用洛铂预防胃癌术后肿瘤复发的安全性和有效性。方法采用随机对照研究设计,研究对象为进展期胃癌的80例住院患者。对照组:40例采用腹腔镜胃癌D2根治性手术;实验组:40例采用腹腔镜胃癌D2根治术,术中将100 mg洛铂稀释在250 ml温生理盐水中,夹闭腹腔引流管6 h。术中两组患者均取腹腔冲洗液进行脱落癌细胞检测,术后观察患者生命体征,胃肠道通气时间、不良反应及手术并发症情况。随访1、2年肿瘤复发情况。结果两组患者均顺利完成了胃癌D2根治术,实验组和对照组患者术后并发症发生率分别为17.5%和15%,并发症包括肺部感染、腹腔感染、肠梗阻、切口感染,均无统计学意义。实验组及对照组胃肠道通气时间、住院时间指标,术中脱落癌细胞检测情况不具有统计学意义(P>0.05)。对照组出现的白细胞减少、血小板减少情况,具有统计学意义(P<0.05)。实验组术后1年复发率为5%;对照组术后1年复发率为10%,实验组术后2年复发率为22.5%;对照组术后2年复发率为37.5%;两组患者术后1年、2年复发率比较具有统计学意义(P<0.05)。结论术中应用洛铂腹腔灌洗化疗预防胃癌术后肿瘤复发是安全有效的。Objective To explore the safety and efficacy of lobaplatin in preventing postoperative recurrence of gastric cancer.Methods A randomized controlled study design was used.The subjects were 80 patients diagnosed with advanced gastric cancer.The control group:40 patients underwent laparoscopic D2 radical surgery for gastric cancer;the experimental group:40 patients underwent laparoscopic D2 radical gastrectomy with 100mg lobaplatin diluted in 250ml warm normal saline,and the abdominal drainage tube was clipped for 6h.Intraoperative peritoneal rinsing fluid was taken for the detection of shedding cancer cells in both groups,and postoperative vital signs,gastrointestinal ventilation time,adverse reactions and surgical complications were observed.The tumor recurrence was followed up for 1 and 2 years.Results Both groups successfully completed D2 radical gastrectomy for gastric cancer.Postoperative complications were 17.5%in the experimental group and 15%in the control group,including pulmonary infection,abdominal infection,intestinal obstruction and incision infection.There had no statistically significant difference in gastrointestinal ventilation time,hospital stay time and detection of cancerous cells in the experimental group and the control group(P>0.05).The leukopenia and thrombocytopenia occurred in the control group,there had statistically significant difference(P<0.05).The 1-year recurrence rate was 5%in the experimental group,and of the control group was 10%.The 2-year recurrence rate of the experimental group was 22.5%,and of the control group was 37.5%.The 1,2-year recurrence rates of the 2 groups had statistically significant difference(P<0.05).Conclusion The intraperitoneal lavage chemotherapy with lobaplatin is safe and effective in preventing postoperative recurrence of gastric cancer.
