HSP27、KLF5对DHA方案治疗儿童急性髓系白血病疗效的评估价值

摘　　要：目的探究热休克蛋白27(HSP27)、Kruppel样因子5(KLF5)对DHA方案(高三尖杉酯碱+盐酸柔红霉素+阿糖胞苷)治疗儿童急性髓系白血病(AML)疗效的评估价值。方法选择行DHA方案治疗的AML患儿75例,检测治疗前后患儿血清HSP27、KLF5表达,并分析二者对患儿治疗效果的评估价值。结果血清HSP27、KLF5水平在不同危险程度AML患儿之间存在显著差异,且随危险程度增加,患儿血清HSP27、KLF5水平逐渐升高(P<0.05)。治疗后AML患儿血清HSP27、KLF5水平较治疗前降低(P<0.05);治疗有效患儿血清HSP27、KLF5水平低于无效患儿(P<0.05)。建立HSP27、KLF5诊断AML治疗有效的ROC曲线,其AUC分别为0.717、0.758,两者联合诊断AML治疗有效的AUC为0.886,高于单一指标(P<0.05)。结论HSP27、KLF5表达水平与AML患儿危险程度有关,且对DHA方案治疗效果具有一定的临床评估价值。Objective To explore the evaluation value of heat shock protein 27(HSP27)and Kruppel-like factor 5(KLF5)on the efficacy of DHA regimen(homoharringtonine+daunorubicin+cytarabine)in the treatment of children with acute myeloid leukemia(AML).Methods 75 children with AML who were treated with DHA regimen were selected.The serum HSP27 and KLF5 expressions of the children were detected before and after the treatment,and the evaluation value of the two on the treatment effect of the children was analyzed.Results There were significant differences in serum HSP27 and KLF5 levels among children with AML at different risk levels,and with the increase in risk,the serum HSP27 and KLF5 levels gradually increased(P<0.05).After treatment,serum HSP27 and KLF5 levels in children with AML were lower than before treatment(P<0.05).The serum HSP27 and KLF5 levels of children with effective treatment were lower than those of ineffective children(P<0.05).The ROC curve of HSP27 and KLF5 for the diagnosis and treatment of AML was established,and their AUC were 0.717 and 0.758,respectively;the effective AUC of the two combined diagnoses for AML treatment was 0.886,which was higher than a single indicator(P<0.05).Conclusion The expression levels of HSP27 and KLF5 are related to the risk of children with AML,and have certain clinical evaluation value for the treatment effect of DHA regimen.
