摘 要:目的 探讨食管鳞状细胞癌组织微小RNA(miR)-145、miR-765表达变化及意义。方法 选择食管鳞状细胞癌患者80例,应用实时荧光定量聚合酶链反应(qRT-PCR)法检测癌组织及癌旁组织中miR-145、miR-765表达;应用免疫组化法检测组织中增殖细胞核抗原,计算阳性细胞百分数作为增殖指数(PI);应用Td T介导的脱氧尿苷酸缺口末端标记(TUNEL)法测算凋亡指数(AI);分析食管鳞状细胞癌组织中miR-145、miR-765表达与患者临床病理特征的关系;随访36个月,观察不同miR-145、miR-765表达患者生存情况;分析患者死亡的危险因素。结果食管鳞状细胞癌组织中miR-145表达、AI低于癌旁组织,miR-765表达、PI高于癌旁组织(P均<0.05)。Pearson相关分析显示,食管鳞状细胞癌组织中miR-145表达与PI呈负相关(r=-0.522,P <0.05),与AI呈正相关(r=0.578,P <0.05);miR-765表达与PI呈正相关(r=0.602,P <0.05),与AI呈负相关(r=-0.511,P <0.05)。miR-145、miR-765表达与组织分化程度、TNM分期有关(P均<0.05)。经Log-rank检验发现,miR-145高表达组生存率高于miR-145低表达组,miR-765低表达组生存率高于miR-765高表达组(P <0.05)。Cox分析结果显示,低分化、TNM分期Ⅲ期、miR-145低表达、miR-765高表达是食管鳞状细胞癌患者死亡的危险因素(P均<0.05)。结论 食管鳞状细胞癌组织中miR-765异常高表达、miR-145异常低表达,二者均与癌细胞增殖有关;检测二者表达有助于食管鳞状细胞癌病情判断及预后评估。Objective To investigate the expression changes and significance of microRNA(miR)-145 and miR-765 in the esophageal squamous cell carcinoma tissues.MethodsEighty patients with esophageal squamous cell carcinoma were selected and the expression levels of miR-145 and miR-765 in the cancer and adjacent tissues were detected by real-time fluorescence quantitative polymerase chain reaction(qRT-PCR).Immunohistochemical staining was used to detect tissue proliferative nuclear antigen and the percentage of positive cells was calculated as proliferation index(PI).Apoptosis index(AI)was detected by TdT-mediated deoxyuridine notch end labeling(TUNEL).The relationships between the miR-145 and miR-765 expression in esophageal squamous cell carcinoma tissues and the clinicopathological characteristics of patients were analyzed.At 36 months after follow-up,the survival of patients with different miR-145 and miR-765 expression were observed.The risk factors for death of patient were analyzed.Results The miR-145 expression and AI in the esophageal squamous cell carcinoma tissues were significantly lower than those in the paracancerous tissues,and the miR-765 expression level and PI were significantly higher than those in the paracancerous tissues(all P<0.05).Pearson correlation analysis showed that the miR-145 expression was negatively correlated with PI(r=-0.522,P=0.023),and positively correlated with AI(r=0.578,P=0.011).The miR-765 expression level was positively correlated with PI(r=0.602,P=0.000),and negatively correlated with AI(r=-0.511,P=0.017).The miR-145 and miR-765 expression levels were related to the tissue differentiation degree and TNM stage(all P<0.05).Log-rank test showed that the survival rate of the high miR-145 expression group was significantly better than that of the low miR-145 expression group,and the survival rate of the low miR-765 expression group was significantly better than that of the high miR-765 expression group(both P<0.05).Results of Cox analysis showed that low differentiation,TNM stage III,low
