右丙亚胺联合经外周静脉穿刺的中心静脉导管置管在乳腺癌化疗患者中的应用效果

摘　　要：目的探讨右丙亚胺联合经外周静脉穿刺的中心静脉导管(PICC)置管在乳腺癌化疗患者中的应用效果。方法将122例乳腺癌患者根据不同的治疗方法分为GP方案(吉西他滨+顺铂)组和右丙亚胺组,每组61例。GP方案组进行GP方案化疗,右丙亚胺组在GP方案组基础上给予右丙亚胺。比较两组患者焦虑自评量表(SAS)和抑郁自评量表(SDS)评分,分析血管内皮生长因子(VEGF)A、B水平,比较两组患者的生活质量核心量表(QLQ-C30)评分和癌症治疗功能评价系统(FACT)-B评分。结果治疗后,右丙亚胺组患者SDS、SAS评分均明显低于GP方案组(P﹤0.01),VEGFA、VEGFB水平均明显低于GP方案组(P﹤0.01),QLQ-C30量表及FACT-B量表评分均明显高于GP方案组(P﹤0.01),不良反应总发生率明显低于GP方案组(P﹤0.01)。结论在乳腺癌化疗患者中应用右丙亚胺联合PICC置管,可以降低患者的不良情绪,改善患者生活质量及躯体功能,有效抑制了肿瘤血管的新生,降低了肿瘤的生长及转移能力。Objective To investigate the efficacy of dexrazoxane combined peripherally inserted central venous catheter(PICC)placement for postoperative chemotherapy of breast cancer.Method A total of 122 breast cancer patients were divided into the GP(gemcitabine+cisplatin)group(n=61,treated with GP conventional chemotherapy)and dexra-zoxane group(n=61,treated with dexrazoxane+GP conventional chemotherapy)according to different treatment meth-ods.The scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS),the levels of vascular endothelial growth factor(VEGF)A and VEGFB,the scores of quality of life questionnaire-core 30(QLQ-C30)and functional as-sessment of cancer therapy(FACT)-B of two groups were compared.Result After treatment,SDS and SAS scores in the dexrazoxane group were significantly lower than those of the GP group(P<0.01),the levels of VEGFA and VEGFB in the dexrazoxane group were significantly lower than those of the GP group(P<0.01),the scores of QLQ-C30 and FACT-B in the dexrazoxane group were significantly higher than those of the GP group(P<0.01),and the total incidence of adverse reaction in the dexrazoxane group was significantly lower than that of the GP group(P<0.01).Conclusion Dexrazoxane+PICC placement for postoperative chemotherapy in patients with breast cancer can reduce the adverse mood,improve the quality of life and somatic function,effectively inhibit the neogenesis of tumor vessels,and reduce the growth and metastasis of tumors.
