摘　　要：目的评估超细内镜在消化道严重狭窄的评估和治疗中的应用价值。方法回顾性分析2009年10月1日至2019年10月1日在我科行消化内镜检查明确诊断为消化道严重狭窄并更换超细内镜检查患者的临床内镜特征。结果在10年时间内,我科共行内镜检查及治疗108 176例次,其中诊断为消化道严重狭窄并更换超细内镜检查者283例,其中上消化道234例(82.7%),下消化道49例(17.3%)。消化道狭窄病因以恶性肿瘤最多,共157例(55.5%)。超细内镜在上、下消化道严重狭窄通过率分别为91.5%、55.1%,通过后发现其他病变比例分别为12.6%、22.2%。在成功通过的上消化道狭窄病例中,有24例食管狭窄患者利用超细内镜成功辅助完成内镜下治疗。结论超细内镜在消化道严重狭窄的患者中应用十分重要。不仅能为患者提供完整的内镜诊断,同时为内镜下治疗创造条件。Objective To assess the application value of ultrathin endoscopy for the evaluation and treatment of advanced gastrointestinal strictures. Methods The clinical and endoscopic data of patients diagnosed as advanced gastrointestinal strictures and underwent a second procedure with a ultrathin endoscopy in the same session were retrospectively analyzed from Oct.1 st, 2009 to Oct. 1 st, 2019. Results During a 10 year study period, 234 patients(82.7%) at upper and 49 ones(17.3%) at lower endoscopy were detected with advanced gastrointestinal stricture among 108 176 endoscopy. The most common etiologies was malignant tumor(157 cases) which account for 55.5% of total patients. A complete endoscopic examination was successful with ultrathin endoscopy in 214 patients(91.5%) with upper and in 27 patients(55.1%) with lower gastrointestinal strictures. Moreover, new lesions were detected in 27 patients(12.6%) with upper and 27 patients(55.1%) with lower gastrointestinal strictures from the distal end of the strictures session. An interventional procedure was also carried out in 24 patients with the assistance of ultruthin endoscopy.Conclusion Ultrathin endoscopy is a useful tool for the evaluation of patients with advanced gastrointestinal strictures. It provides a complete diagnostic endoscopy in most patients and gives an opportunity for therapeutic endoscopic procedures.
