摘  要:目的:分析不同正畸力值对牙髓活力的影响,探讨基质金属蛋白酶9(MMP-9)蛋白表达与不同正畸力值牙髓活力的相关性。方法:选取我院2019年12月—2020年12月共收治的60例接受正畸治疗的患者作为研究对象,将患者随机分为轻力组和重力组,每组30例。轻力组患者对双侧第一前磨牙施加50g压低力,重力组施加300g压低力。对比两组患者不同时间的牙髓活力,分析不同正畸力值与不同时间牙髓活力的相关性。随后分析不同正畸力下牙髓组织中MMP-9灰度积分对比,并分析MMP-9蛋白表达与不同正畸力值牙髓活力的相关性。结果:重力组患者从加力5周开始受试者牙齿对于EPT无反应,两组患者EPT初始值、1周、2周、3周和4周EPT测试值对比,重力组高于轻力组,对比差异显著(P<0.05);Spearman相关分析结果显示:不同正畸力值与初始值、正畸1周、正畸2周和正畸3周的牙髓活力呈正相关(P<0.05);轻力组与重力组MMP-9数值从1周到4周逐渐增加,从4周到12周开始回落,轻力组与重力组患者第1周MMP-9表达对比无明显差异(P>0.05),在4周、8周和12周的MMP-9表达重力组高于轻力组,差异具有统计学意义(P<0.05);Spearman相关分析结果显示:轻力组患者加力时间1周MMP-9蛋白表达与牙髓活力无明显相关性(P>0.05),加力时间4周、8周和12周MMP-9蛋白表达与牙髓活力呈正相关(P<0.05);重力组患者加力时间1周、8周和12周的MMP-9蛋白表达与牙髓活力无明显相关性(P>0.05),加力时间4周MMP-9蛋白表达与牙髓活力呈正相关(P<0.05)。结论:应用较大龈向压低力在短期作用下会导致牙髓活力降低,MMP-9表达量会随着压力时间的变化而变化,MMP-9表达与一定时间的不同正畸力牙髓活力具有一定相关性,由此证明MMP-9可能参与龈向压低力作用下的牙髓组织改建过程,而且正畸力值越大MMP-9的表达量越多。Objective:To analyze the effect of different orthodontic forces on pulp vitality,and to explore the correlation between the expression of matrix metalloproteinase-9(MMP-9)and pulp vitality.Methods:60 cases of orthodontic patients in our hospital from December 2019 to December 2020 were randomly divided into light force group and gravity group,30 cases in each group.In the light force group,50g force was applied to the bilateral first premolars,while in the gravity group,300g force was applied.The pulp vitality of the two groups at different time was compared,and the correlation between different orthodontic force values and pulp vitality at different time was analyzed.Then,the gray scale score of MMP-9 in pulp tissue was compared under different orthodontic forces,and the correlation between the expression of MMP-9 protein and pulp vitality under different orthodontic forces was analyzed.Results:The teeth of the gravity group had no response to EPT from the 5th week of the force application,and the differences of the initial value,1 week,2 weeks,3 weeks and 4 weeks of EPT between the two groups were significant,the gravity group was higher than the light force group,and the differences were statistically significant(P<0.05).Spearman correlation analysis showed that different orthodontic force values were positively correlated with initial value,1 week,2 weeks and 3 weeks of orthodontic treatment.There was no significant difference in the expression of MMP-9 between the light force group and the gravity group in the first week(P>0.05).The expression of MMP-9 of the gravity group was higher than that in the light force group at 4 weeks,8 weeks,12weeks(P<0.05).The results of Spearman correlation analysis showed that there was no significant correlation between the expression of MMP-9 protein and pulp vitality in the light force group after 1 week of loading(P>0.05),and there was a positive correlation between the expression of MMP-9 protein and pulp vitality after 4,8 and 12 weeks of loading(P<0.05);there was no sig
