Tg及TSGF在甲状腺癌诊断中的应用价值分析

摘　　要：目的探讨甲状腺球蛋白(Tg)、肿瘤特异性生长因子(TSGF)在甲状腺癌诊断中的意义。方法选取2019年1月至2020年12月茂名市人民医院(我院)收治的150例甲状腺癌患者为恶性组和150例良性甲状腺病变患者为良性组,另选取同一时期于我院进行健康检查的150名受检者为对照组,分别检测并比较三组Tg和TSGF;按照病理学检测结果将恶性组患者分为Ⅰ期(44例)、Ⅱ期(39例)、Ⅲ期(40例)、Ⅳ期(27例),比较不同分期甲状腺癌患者的血清Tg、TSGF水平;分析血清TSGF、Tg阳性率与颈部淋巴结转移的关系。结果良性组的血清TSGF、Tg水平高于对照组,恶性组患者的血清TSGF、Tg水平高于良性组(P<0.05);Ⅰ期与Ⅱ期血清Tg、TSGF水平比较,差异无统计学意义(P>0.05),Ⅳ期患者的血清Tg、TSGF水平显著高于Ⅰ期、Ⅱ期、Ⅲ期患者;Ⅲ期患者的血清Tg、TSGF水平显著高于Ⅰ期、Ⅱ期患者(P<0.05);出现有颈部淋巴结转移的甲状腺癌患者其血清TSGF、Tg阳性率更高(P<0.05)。结论对血清Tg和TSGF水平进行检测,有利于早期发现甲状腺癌,并为疾病的病程、是否出现颈部淋巴结转移提供判定依据,在甲状腺癌的早期诊断中具有非常重要的临床意义。Objective To explore the significance of thyroglobulin(Tg)and tumor-specific growth factor(TSGF)in the diagnosis of thyroid cancer.Methods A total of 150 patients with thyroid cancer admitted to Maoming People's Hospital(our hospital)from January 2019 to December 2020 were selected as the malignant group and 150 patients with benign thyroid lesions as the benign group,and another 150 subjects who underwent health examination in our hospital during the same period were selected as the control group.The Tg and TSGF were detected and compared among the three groups.The patients in the malignant group were divided into stageⅠ(44 cases),stageⅡ(39 cases),stageⅢ(40 cases)and stageⅣ(27 cases)according to the pathological test results,and the serum Tg and TSGF levels of patients in different stages of thyroid cancer were compared.The relationship between the positive rates of serum TSGF and Tg and lymph node metastasis in the neck was analyzed.Results The serum TSGF and Tg levels in the benign group were higher than those in the control group,and those in the malignant group were higher than those in the benign group(P<0.05).There was no statistically significant difference in serum Tg and TSGF levels between stageⅠand stageⅡ(P>0.05),and the serum Tg and TSGF levels in stageⅣpatients were significantly higher than those in stageⅠ,ⅡandⅢpatients.The serum Tg and TSGF levels in stageⅢpatients were significantly higher than those in stageⅠandⅡpatients(P<0.05).The positive rates of serum TSGF and Tg were higher in patients with thyroid cancer who presented with lymph node metastasis in the neck(P<0.05).Conclusion The detection of serum Tg and TSGF levels is beneficial for the early detection of thyroid cancer and provides a basis for determining the course of the disease and the presence of lymph node metastasis in the neck,which is of great clinical significance in the early diagnosis of thyroid cancer.
