加味白头翁汤治疗Ⅲ型慢性前列腺炎的疗效及其对PSEP的影响

摘　　要：目的:探讨加味白头翁汤治疗Ⅲ型慢性前列腺炎(CP)的临床疗效及其对前列腺小体外泄蛋白(PSEP)的影响。方法:将110例Ⅲ型CP患者随机分为治疗组和对照组,每组各55例。治疗组予自拟加味白头翁汤治疗,对照组予宁泌泰胶囊治疗。观察2组综合疗效及治疗前后PSEP值、前列腺液(EPS)常规中白细胞计数(WBC)、慢性前列腺炎症状指数(NIH-CPSI)评分。结果:总有效率治疗组为83.64%(46/55),明显高于对照组的70.91%(39/55),差异有统计学意义(P<0.05)。治疗后2组PSEP值、EPS中WBC计数情况及NIH-CPSI各项指标评分均较治疗前明显改善,且治疗组改善幅度大于对照组,差异均有统计学意义(P<0.05)。结论:加味白头翁汤可以有效治疗Ⅲ型CP,检测尿液中的PSEP可以作为评估Ⅲ型CP临床疗效的指标,值得推广。Objective: To investigate the clinical effect of Jiawei Baitouweng decoction in the treatment of type Ⅲ chronic prostatitis(CP) and its effect on prostatic exosomal protein(PSEP).Methods: A total of 110 patients with type Ⅲ CP were randomly divided into treatment group and control group, with 55 patients in each group.The patients in the treatment group were given self-made Jiawei Baitouweng decoction, and those in the control group were given Ningmitai capsules.The two groups were observed in terms of overall response, PSEP,white blood cell count(WBC) in expressed prostatic secretion(EPS),and National Institutes of Health Chronic Prostatitis Symptom Index(NIH-CPSI) score before and after treatment.Results: The treatment group had a significantly higher overall response rate than the control group [83.64%(46/55)vs70.91%(39/55),P<0.05].After treatment, both groups had significant improvements in PSEP,WBC in EPS,and NIH-CPSI score, and the treatment group had significantly greater improvements than the control group(P<0.05).Conclusion: Jiawei Baitouweng decoction can effectively treat type Ⅲ CP,and measurement of PSEP in urine can be used as an index for evaluating the clinical outcome of type Ⅲ CP.This treatment holds promise for clinical application.
