摘 要:目的观察姜黄素对脑缺血再灌注损伤沙土鼠模型的保护作用,探讨其作用机制。方法双侧颈总动脉夹闭手术法建立脑缺血再灌注损伤沙土鼠模型,随机分为假手术组、模型组、阳性对照尼莫地平组(2 mg·kg^(-1))、姜黄素高剂量和低剂量组(50和25 mg·kg;),建模前1w开始腹腔注射给药,再灌注6 h内记录各组神经症状并做卒中评分,再灌注6 h后免疫组化法测定脑组织海马CA1区核因子-κ B(NF-κ B)、基质金属蛋白酶9 (MMP-9)和细胞间黏附分子1 (ICAM-1)的表达。结果与模型组卒中评分(19.1分)相比,姜黄素高、低剂量组卒中评分降低为16.2、14.0分(P<0.05或P<0.01);姜黄素高、低剂量可明显下调沙土鼠脑海马组织NF-κ B阳性表达为(42.08±14.05)、(49.36±9.81),与模型组(IOD值:64.55±13.07)相比组间差异有统计学意义(P<0.01);与模型组IOD值(39.84±7.05)相比,姜黄素高、低剂量可明显下调沙土鼠脑海马组织ICAM-1阳性表达为(23.79±7.38)、(29.12± 10.16),组间差异有统计学意义(P<0.05或P<0.01);姜黄素高剂量脑海马组织MMP-9的表达IOD值为(14.21± 3.99),与模型组(19.38±4.09)相比差异有统计学意义(P<0.05)。结论姜黄素预处理对脑缺血再灌注损伤沙土鼠具有脑保护作用,可能与抑制炎症因子和黏附分子表达,减轻脑部炎症反应,降低血-脑脊液屏障的通透性,缓解缺血区脑组织的损伤程度等有关。Objective To explore the protective effect of curcumin(CUR) pretreatment on gerbils with cerebral ischemia/reperfusion injury(CI/RI) and its preliminary mechanism.Methods The gerbil model of cerebral ischemia/reperfusion injury was prepared by a surgery of bilateral common carotid occlusion followed by6-hour reperfusion.Gerbils were randomly divided into model,positive control(nimodipine 2 mg·kg^(-1)),CUR high dose(50 mg·kg;) and low dose(25 mg·kg;) and sham groups before the model preparation,and drugs were injected intraperitoneally for 7 days pre-surgery.During 6-hour reperfusion,the neurological symptoms of gerbils in all groups were evaluated by stroke scores.After 6-hour reperfusion,hippocampi tissues of gerbils with cerebral ischemia/reperfusion injury were collected and the levels of nuclear factor-κ B(NF-κ B),matrix metalloproteinases-9(MMP-9) and intercellular cell adhesion molecule-1(ICAM-1) were detected by immunohistochemistry.Results Compared with stroke scores(19.1) of model group,those in CUR pretreatment groups(16.2 and 14.0,respectively) were significantly improved(P<0.05 or P<0.01).Compared with NF-κ B expression(64.55±13.07) in hippocampi tissue of model group,those in CUR pretreatment groups(49.36±9.81 and 42.08±14.05,respectively) were markedly reduced(P<0.01).Compared with ICAM-1 expression(39.84±7.05) in hippocampi tissue of model group,those in CUR pretreatment groups(29.12±10.16 and 23.79±7.38,respectively) were significantly reduced(P<0.05 or P<0.01).CUR(50 mg·kg;) significantly inhibited MMP-9 expression(IOD:14.21±3.99) in hippocampi tissue comparing to that(IOD:19.38±4.09) of model group(P<0.05).Conclusion CUR pretreatment shows a neuro-protective effect on the brain tissue of CI/RI gerbils,its action maybe related with the inhibition of inflammatory factors and adhesion molecules,relieving the inflammatory response and brain injury and maintaining the integrity of blood-brain barrier,etc.
