摘  要:目的探讨经阴道取出标本的腹腔镜远端胃癌根治术的可行性和安全性。方法回顾性分析2020年5月~2021年4月5例经阴道取出标本的腹腔镜远端胃癌根治术资料。结果5例均顺利完成手术,其中1例合并巨大子宫肌瘤,同时行腹腔镜全子宫+双侧附件切除术。中位手术时间290(205~300)min,术中失血量30(20~50)ml。无并发症发生。术后1~2天下地活动,2~3天进食流质饮食,术后2天拔除尿管,术后6~7天顺利出院。结论严格把握手术的指征,选择合适的病例,经阴道取出标本的腹腔镜远端胃癌根治术安全可行。Objective To investigate the feasibility and safety of laparoscopic distal gastrectomy with specimen extraction through vagina.Methods Cinical data of 5 patients with distal gastric cancer who underwent laparoscopic radical gastrectomy with specimen extraction through vagina from May 2020 to April 2021 in our hospital were retrospectively analyzed.Results All the 5 patients were successfully received laparoscopic distal gastrectomy and transvaginal specimen extraction.One of them was accompanied with huge hysteromyoma and underwent laparoscopic hysterectomy plus bilateral appendectomy.The median operation time was 290 min(range,205-300 min)and the median intraoperative blood loss was 30 ml(range,20-50 ml).No complications occurred in the 5 patients.The patients went down to the ground on 1-2 days after operation,and took fluid diet on 2-3 days.The catheter was removed 2 days after operation,and the patients were discharged 6-7 days after operation.Conclusion It is safe and feasible to carry out laparoscopic distal gastrectomy with transvaginal specimen extraction,as long as strictly grasping the indications of operation and selecting appropriate cases.
