摘  要:目的:探讨轮班护士5-羟色胺(5-HT)转运体基因启动子区(5-HTTLPR)、内含子区(5-HTTVNTR)和5-HT 1A受体(5-HTR1A)启动子区C-1019G基因多态性及其交互作用与睡眠障碍(SD)的关系。方法:采用匹兹堡睡眠质量指数量表(PSQI)对138名轮班制护士进行睡眠质量评定,将入组者分为SD组(59例)和非SD组(79例);对两组5-HTTLPR、5-HTTVNTR和5-HTR1A C-1019G基因多态性进行检测;比较两组各基因型及各等位基因的频率;分析基因-基因交互作用与SD的关联。结果:两组5-HTTLPR和5-HTR1A C-1019G位点基因型及等位基因频率比较差异有统计学意义(P均<0.01);5-HTTLPR位点S等位基因携带者发生SD危险性明显高于L等位基因者(OR=1.244,95%CI:1.067~1.451),5-HTR1A C-1019G位点G等位基因携带者发生SD危险性明显高于C等位基因者(OR=1.428,95%CI:1.246~1.764)。5-HTTLPR和5-HTR1A C-1019G两位点存在交互作用,携带SS/GG基因型SD危险性明显高于LL/CC型(OR=9.429,95%CI:1.023~86.857,P=0.043)。结论:5-HTTLPR和5-HTR1A C-1019G基因多态性与轮班护士SD易感性相关;5-HTTLPR SS基因型/5-HTR1A C-1019G GG基因型者SD易感性显著增加。Objective:To investigate the relationship of serotonin(5-HT)transporter gene promoter region(5-HTTLPR),intron region(5-HTTVNTR)and 5HT 1A receptor(5-HTR1A)promoter region C-1019G gene polymorphisms and their interactions with sleep disturbance(SD)in shift nurses.Method:The Pittsburgh Sleep Quality Index(PSQI)was used to evaluate the sleep quality of 138 nurses engaged in shift work in our hospital.The participants were divided into SD group(59 cases)and non-SD group(79 cases).Detect the gene polymorphisms of 5-HTTLPR,5-HTTVNTR and 5-HTR1A C-1019G in the two groups.The frequency distribution of genotypes and alleles in the two groups was compared,and the association between gene-gene interaction with susceptibility to SD was analyzed.Results:There was significant difference in genotype and allele frequency distribution of 5-HTTLPR and 5-HTR1A C-1019G polymorphisms between the two groups(P<0.05).Individuals with S allele at 5-HTTLPR locus had higher risk of SD than those with L allele(OR=1.244,95%CI:1.067-1.451)and individuals with G allele at 5-HTR1A C-1019G locus had higher risk of SD than those with C allele(OR=1.428,95%CI:1.246-1.764).There was an interaction between 5-HTTLPR and 5HTR1A C-1019G,and individuals with SS/GG genotype had higher risk of SD than those with LL/CC genotype(OR=9.429,95%CI:1.023-86.857,P=0.043).Conclusion:5-HTTLPR and 5-HTR1A C-1019G gene polymorphisms were associated with SD susceptibility of shift nurses,and those who carried 5-HTTLPR SS genotype/5-HTR1A C-1019G GG genotype were significantly more susceptible to SD.
