连云港市2019年严重精神障碍患者社区随访管理现状

摘　　要：目的:分析连云港市2019年严重精神障碍患者社区随访管理情况。方法:采用描述性分析方法,对2019年连云港市严重精神障碍信息系统中登记的严重精神障碍(包括精神分裂症、分裂情感性障碍、偏执性精神病、双相障碍、癫痫所致精神障碍、精神发育迟滞伴发精神障碍)患者一般人口学资料及社区管理资料进行统计,并与2018年相应数据进行比较。结果:截至2019年,全市严重精神障碍信息系统在册患者17975例;诊断为精神分裂症占71.77%;报告患病率0.398%。在管患者17138例,管理率95.34%;规范管理率93.53%,服药率85.18%,规律服药率57.6%,精神分裂症患者服药率89.61%,精神分裂症患者规律服药率64.27%,面访率为83.63%;与2018年相比,严重精神障碍患者管理率、服药率、规律服药率、精神分裂症患者服药率、精神分裂症患者规律服药率、面访率分别增加0.61%、2.63%、11.61%、2.35%、13.24%、7.15%;报告患病率、规范管理率分别下降0.015%、0.75%。结论:2019年连云港市在册的严重精神障碍患者多为精神分裂症;社区随访管理情况较好,且较2018年有所改善;进一步提高对严重精神障碍患者救助支持及提高精神分裂症患者规律服药率应是社区随访管理的重点。Objective:To analyze the community-based management of patients with severe mental disorders in Lianyungang in 2019.Method:Descriptive analysis was used to analyze the registration,management and treatment of patients in the information system of severe mental disorders(schizophrenia,schizoaffective disorder,paranoid mental disorders,bipolar disorder,mental disorders due to epilepsy and mental retardation comorbidity with mental disorder)in Lianyungang in 2019,and the data were compared with those reported in 2018.Results:By the end of 2019,17975 patients with severe mental disorders were registered in the information system.71.77% of them were schizophrenia patients,reported prevalence is 0.398%;17138 patients were under management,management rate was 95.34%;standardized management rate was 93.53%,medication rate was 85.18%,regular medication rate was 57.6%,medication rate of schizophrenics was 89.61%,regular medication rate of schizophrenia was 64.27%,and interview rate was 83.63%.Compared with 2018,the management rate,medication rate,regular medication rate,medication rate of schizophrenics,regular medication rate of schizophrenics and interview rate of patients with severe mental disorders respectively increased by 0.61%,2.63%,11.61%,2.35%,13.24% and 7.15%.Reported prevalence and standardized management rate respectively decreased by 0.015% and 0.75%.Conclusion:In 2019,the management and treatment situation of patients with severe mental disorders was improved in Lianyungang,the interview compliance was improved,and the condition was further controlled.However,it is still necessary to strengthen the communication and cooperation between professional medical institutions and primary medical staff of psychiatric disorder management to increase the prevalence of reports and strengthen the standardized follow-up frequency of grass-root fine prevention personnel.
