摘 要:目的分析脂肪非典型钙黏蛋白1(FAT1)在肺腺癌不同发展阶段中的表达及其与浸润性肺腺癌临床病理学特征的关系,探讨其在肺腺癌发生、发展中的作用。方法采用组织芯片及免疫组化方法检测329例癌旁组织、21例非典型腺瘤样增生(AAH)组织,130例原位腺癌组织、178例浸润性腺癌组织中FAT1的表达,分析其与临床病理特征间的关系。结果FAT1蛋白在4种组织内呈不同程度表达,在癌旁组织、AAH、原位腺癌、浸润性腺癌的表达阳性率分别为31.9%(105/329)、100%(21/21)、100%(130/130)和100%(178/178),其表达强度呈递增趋势。在原位腺癌与浸润性肺腺癌之间FAT1的表达无明显差异。FAT1在不同类型浸润性肺腺癌中的表达率以附壁型最高,腺泡型及乳头型,微乳头型及实性型最弱(P<0.05),分别为87.1%(27/31)、80.1%(97/120)、55.6%(15/27),并且与脉管侵犯及胸膜侵犯等临床病理特征相关(P<0.05)。结论FAT1从正常肺组织到AAH再到原位腺癌的发展过程中表达强度逐渐增强,可能是肺腺癌发生及发展过程中潜在的癌基因。在实性及微乳头亚型中FAT1表达较低且患者生存时间最短,FAT1可能提示预后较差。因此,我们推测FAT1在肺浸润性腺癌的发生及发展过程中具有一定作用,可能为肺浸润性腺癌的发生机制研究提供新的思路。Objective To analyze the expression of Fatty Acid Desaturase-1(FAT1)in different stages of lung adenocarcinoma and the relationship with the clinicopathological characteristics of invasive adenocarcinoma,and explore its role in the occurrence and development of lung adenocarcinoma.Methods We used tissue microarray and immunohistochemical method to detect the expression of FAT1 in 329 cases of paracarcinoma tissues,21 cases of atypical adenomatous hyperplasia(AAH)tissues,130 cases of adenocarcinoma in situ tissues,and 178 cases of invasive adenocarcinoma tissues.and analyzed the relationship of clinicopathological features.Results FAT1 protein was expressed in all tissues,and the expression rates of paracancerous tissues,AAH,adenocarcinoma in situ,and invasive adenocarcinoma were 35.0%(85/245),100%(12/12),and 100%,respectively(132/132)and 100%(83/83),The expression of FAT1 from nomal tissues to AAH to adenocarcinoma in situ showed an increasing trend.There was no significant difference in the expression of FAT1 between adenocarcinoma in situ and invasive adenocarcinoma.The expression rate of FAT1 is highest in the mural-growing type,the micropapillary type and the solid type are the lowest(P<0.05),respectively 87.1%(27/31),80.1%(97/120),55.6%(15/27),and it is related to clinicopathological features such as vascular invasion and pleural invasion(P<0.05).Conclusion The expression of FAT1 gradually increases during the development process from normal lung tissue to AAH to adenocarcinoma in situ and invasive adenocarcinoma,and it may be a potential oncogene in the occurrence and development of lung adenocarcinoma.In solid and micropapillary subtypes,FAT1 expression is lower and the survival time is the shortest.Therefore,we speculate that FAT1 is involved in the occurrence and development of lung invasive adenocarcinoma It has a certain effect and may provide new ideas for the study of the mechanism of lung invasive adenocarcinoma.
