摘　　要：目的探究原发性肝细胞肝癌手术切除后复发的独立危险因素.方法回顾性分析2012年1月至2017年1月在安徽省濉溪县医院经手术病理证实的135例原发性肝细胞肝癌患者,将其分为复发组(n=71)和未复发组(n=64),采用单因素和多因素Logistic回归分析确定肝癌手术切除后复发的独立危险因素.结果单因素分析显示肿瘤直径、中性粒细胞-淋巴细胞比值、血小板-淋巴细胞比值、术中输血、门脉癌栓、糖尿病、肿瘤包膜是影响肝癌术后复发的危险因素(P<0.05),进一步行多因素Logistic回归分析显示中性粒细胞-淋巴细胞比值、门脉癌栓和糖尿病是影响肝癌术后复发的独立危险因素.结论肝癌术后复发是多种因素造成的,而高中性粒细胞-淋巴细胞比值、门脉癌栓和糖尿病是原发性肝细胞肝癌外科手术后复发的独立危险因素.Objective To explore the independent risk factors of postoperative recurrence of patients with hepatocellular carcinoma(HCC).Methods A retrospective analysis was performed on 135 patients with primary HCC confirmed by surgery and pathology in our hospitals from January 2012 to January 2017.All patients were divided into two groups including recurrence-positive(n=71)and recurrence-negative(n=64).Uni-and multi-variate regression analysis were conducted to explore the independent risk factors of early postoperative recurrence of primary HCC.Results At univariate analysis,tumor size,the neutrophil-lymphocyte rate,and portal vein tumor thrombus are risk factors of postoperative recurrence of primary HCC(P<0.05).Then,multivariate analysis was further performed,showing higher neutrophil-lymphocyte rate,portal vein tumor thrombus and diabetes are independent risk factors of postoperative recurrence of primary HCC.Conclusions HCC recurrence after resection is caused by many factors,the higher NLR value,portal vein tumor thrombus and diabetes may be considered as predictive indicators of early recurrence primary HCC after operation.
