成年人开[牙合]患者正畸治疗对颞下颌关节的影响

摘　　要：目的通过MRI观察正畸对成人开[牙合]患者颞下颌关节位置及髁突高度的影响。方法对纳入研究的12例进行正畸治疗的开牙合患者的颞下颌关节间隙指数及髁突高度进行回顾性研究。对12例患者24侧的颞下颌关节在正畸前后进行MRI扫描,在经轴位断面上髁突最大内外径中点观察矫治前后的关节间隙指数及髁突高度变化。并对正畸治疗前后结果进行统计分析。结果髁突高度由(5.770±0.220)mm减小为(5.753±0.239)mm,但差异无统计学意义(t=-0.99,P>0.05)。关节间隙指数由2.882±4.390减小为2.716±4.400,但差异无统计学意义(t=-2.027,P>0.05)。结论正畸对成年人开[牙合]患者颞下颌关节的位置及髁突高度无明显改变,正畸仅改善患者的咬合关系,对颞下关节无明显影响。Objective To investigate the effects of orthodontic treatment on the position of temporomandibular joint and the height of condylar process in adult patients with open bite.Methods A total of 12 adult patients with open bite who underwent MRI scanning examination and orthodontic treatment in our hospital were enrolled in the study.The temporomandibular joint space index(TMJ)and condylar height were retrospectively analyzed.And the changes of joint space index and condyle height were observed at the midpoint of the maximum inner and outer diameter of the epicondyle on the axial section before and after orthodontic treatment.The orthodontic treatment effects were statistically analyzed.Rseults After treatment,the condyle height was decreased from(5.770±0.220)mm to(5.753±0.239)mm,but there was no significant diffence before treatment and aftr treatment(P>0.05).The joint space index was decreased from 2.882±4.390 to 2.716±4.400,however,there was no significant diffence before treatment and aftr treatment(P>0.05).Conclusion There are no significant changes in the position of temporomandibular joint and the height of condyle in adult patients with open bite,and orthodontic treatment only improves the occlusal correlation,but has no significant effects on the inferior temporomandibular joint.
