摘　　要：目的探讨外周血游离线粒体DNA定量水平在结直肠癌患者预后判断中的价值。方法选择2017年2月至2019年9月本院收治的结直肠癌患者80例为研究对象。留取入组者晨起空腹肘静脉血5 mL及手术切取肿瘤组织、癌旁组织各25 mg,提取基因组DNA,并行PCR检测。比较不同分化程度(低分化、中分化及高分化)者线粒体mtDNA水平;比较不同部位(肿瘤组织、癌旁组织及正常组织)线粒体mtDNA水平;比较是否合并淋巴结转移线粒体mtDNA水平。结果不同分化程度者线粒体mtDNA水平比较中低分化者低于中分化低者于高分化者,差异有统计学意义(P<0.05);不同部位线粒体mtDNA水平比较中,肿瘤组织低于癌旁组织低于正常组织,差异有统计学意义(P<0.05);合并淋巴结转移者其线粒体mtDNA水平显著低于无淋巴结转移者,差异有统计学意义(P<0.05)。结论线粒体mtDNA表达水平与结直肠癌分化水平、肿瘤侵犯部位及是否合并淋巴结转移存在一定相关性,线粒体mtDNA表达水平越低,分化水平越低,存在淋巴结转移可能性越高。Objective To investigate the value of the quantitative level of free mitochondrial DNA in peripheral blood in the prognosis of colorectal cancer patients.Methods A total of 80 colorectal cancer patients admitted to our hospital from February 2017 to September 2019 were selected as the research objects,5 mL of fasting elbow venous blood was taken from the patients in the stay group in the morning,and 25 mg of tumor tissue and paracancerous tissue were taken from the operation.Genomic DNA was extracted and detected by PCR.The mitochondrial mtDNA levels of patients with different differentiation degrees(low differentiation,medium differentiation and high differentiation)were compared.Mitochondrial mtDNA levels in different parts(tumor tissue,paracancerous tissue and normal tissue)were compared.And the mitochondrial mtDNA level of lymph node metastasis were compared.Results The level of mitochondrial mtDNA in patients with different degrees of differentiation was lower than that in patients with low differentiation and higher differentiation,the difference was statistically significant(P<0.05).The level of mitochondrial mtDNA in tumor tissues was lower than that in adjacent tissues,the difference was statistically significant(P<0.05),while that in patients with lymph node metastasis was significantly lower than that in patients without lymph node metastasis,the difference was statistically significant(P<0.05).Conclusion Mitochondrial mtDNA expression level has certain correlation with colorectal cancer differentiation level,tumor invasion site and lymph node metastasis.The lower the mitochondrial mtDNA expression level,the lower the differentiation level,and the higher the possibility of lymph node metastasis.
