子宫肌瘤组织中SULT1A3、ERα、ERβ水平检测及其对子宫肌瘤切除术预后预测价值分析

摘　　要：目的检测子宫肌瘤组织中硫酸基转移酶基因1A3(SULT1A3)、雌激素受体α(ERα)、雌激素受体β(ERβ)表达水平,并分析其对子宫肌瘤切除术预后的预测价值。方法选取2017年1月至2018年1月湖北省妇幼保健院子宫肌瘤患者335例,均经子宫肌瘤切除术治疗,采用免疫组化染色、Western blot检测子宫肌瘤组织、正常子宫肌层组织中SULT1A3、ERα、ERβ表达;随访3年,根据预后情况将患者分为良好组、不良组,比较良好组、不良组子宫肌瘤组织中SULT1A3、ERα、ERβ表达及其他预后可能影响因素差异;采用Logistic回归分析法分析子宫肌瘤切除术预后影响因素;绘制受试者工作曲线(ROC),分析SULT1A3、ERα、ERβ对子宫肌瘤切除术预后预测价值。结果子宫肌瘤组织中SULT1A3阳性表达率、蛋白相对表达量低于正常子宫肌层组织,ERα、ERβ阳性表达率、蛋白相对表达量高于正常子宫肌层组织(P<0.05);随访过程中335例患者中失访35例,纳入的300例患者中预后良好261例(87.00%),设为良好组,预后不良39例(13.00%),设为不良组;不良组体质量指数(BMI),子宫肌瘤组织ERα、ERβ蛋白表达量,前壁肌瘤、子宫肌瘤直径≥5cm、开腹手术构成比高于良好组,子宫肌瘤组织SULT1A3蛋白表达量低于良好组(P<0.05);Logistic回归分析法显示,BMI、前壁肌瘤、SULT1A3、ERα、ERβ均是子宫肌瘤切除术预后影响因素(P<0.05);ROC结果显示,SULT1A3、ERα、ERβ判断子宫肌瘤切除术后预后不良的最佳截断点分别为0.15、0.80、0.43,三者单独及联合预测子宫肌瘤切除术预后不良的AUC分别为0.701、0.742、0.739、0.795。结论子宫肌瘤组织中SULT1A3表达异常降低,ERα、ERβ表达异常升高,三者联合检测对子宫肌瘤切除术预后预测效能高。Objective To detect the expression levels of sulfate transferase gene 1 A3(SULT1 A3),estrogen receptorα(ERα),and estrogen receptorβ(ERβ)in uterine fibroids tissues,and to analyze the prognosis of hysteromyoma resection predicted value.Methods A total of 335 patients with uterine fibroids in our hospital from January 2017 to January2018 were selected.They were all treated with uterine fibroids.Immunohistochemical staining and Western blot were used to detect the expression of SULT1 A3,ERα,ERβin uterine fibroids and normal myometrium tissues.Followed up for 3 years,according to the prognosis,the patients were divided into good group and poor group.The SULT1 A3,ERα,ERβprotein expression and other prognostic factors in the good group and poor group were compared.Logistic regression analysis was used to analyze the prognostic factors of hysteromyomectomy.Receiver operating curve(ROC)was used to analyze the prognostic value of SULT1 A3,ERα,ERβon the prognosis of hysteromyomectomy.Results The positive expression rate and relative protein expression of SULT1 A3 in uterine fibroids tissue were lower than those in normal myometrium tissues,of which the positive expression rates and relative protein expression of ERαand ERβwere higher than those in normal myometrium tissues(P<0.05).During the follow-up,35 of the 335 patients were lost to follow-up.Of the 300 patients included,261(87.00%)had a good prognosis and were set as the good group,and 39 patients with a poor prognosis(13.00%)were set as the poor group.The BMI,ERα,ERβprotein expression in uterine fibroids tissue,anterior wall fibroids,uterine fibroids diameter≥5 cm,open surgery composition ratio of the poor group were higher than those of the good group,of whihc the SULT1 A3 protein expression in uterine fibroids tissue was lower than that of the good group(P<0.05).Logistic regression analysis showed that BMI,anterior fibroids,SULT1 A3,ERα,and ERβwere all factors affecting the prognosis of hysteromyomectomy(P<0.05).ROC results showed that the best cu
