脑室型颅内压监测在大脑半球大面积梗死患者中的应用

摘　　要：目的探讨脑室型颅内压监测在大脑半球大面积梗死患者救治中的作用及意义。方法回顾性分析2012年1月—2019年10月于我院治疗的大脑半球大面积梗死患者41例。观察组19例患者行脑室型颅内压监测,根据颅内压增高程度给予相应的降颅压治疗,以期达到理想的颅内压控制及维持合理的脑灌注压。对照组22例患者根据临床及影像学表现决定药物治疗和手术方案。比较两组患者的脱水药物用量及预后等。结果与对照组相比,观察组患者的甘露醇用量及使用天数明显减少(P<0.01);观察组预后差占比较对照组低,差异有统计学意义(P<0.05)。结论脑室型颅内压监测可改善大脑半球大面积梗死患者的预后,具有重要的临床意义。Objective To elucidate the effectiveness of ventricle type of intracranial pressure(ICP)monitoring in patients with lar ge hemispheric infarction(LHI).Methods The 41 patients with LHI were admitted in The Affiliated Hospital of Putian University from January 2012 to October 2019.Ventricle type of intracranial pressure monitoring probe implantation was performed in 19 patients(experimental group).According to the ICP condition,the ICP was controlled in the ideal state to maintain reasonable cerebral perfusion pressure(CPP),by using the appropriate treatment to reduce intracranial pressure.The other 22 cases(control group)were treated with routine conservative therapy or surgical treatment,according to the CT/MRI and clinical symptoms.The using dosage of mannitol and the scores of Glasgow outcome scale were compared between two groups.Results The using dosage of mannitol in experimental group was obviously less than control group with statistical significance(P<0.01).As follow up after 6 months,the poor prognosis of the experimental group is lower than that of the control group with statistical significance(P<0.05).Conclusion The ventricle type of ICP monitoring applied in LHI can significantly improve the treatment of LHI and reduce the mortality.
